摘 要:目的探究噻托溴铵吸入粉剂联合小剂量阿奇霉素治疗稳定期支气管扩张症的效果。方法 40例稳定期支气管扩张症患者,采用双色球分为对照组和实验组,每组20例。对照组患者应用常规阿奇霉素治疗,实验组患者应用噻托溴铵吸入粉剂联合小剂量阿奇霉素治疗。观察并对比两组患者治疗前后肺功能指标[用力呼气量(FEV)、用力肺活量占预计值的百分比(FVC%)]水平及疗效。结果治疗后,实验组痊愈11例,有效6例,无效3例,总有效率85.00%;对照组痊愈6例,有效5例,无效9例,总有效率55.00%;实验组总有效率高于对照组,差异具有统计学意义(P<0.05)。治疗前,两组FEV、FVC%对比,差异均无统计学意义(P>0.05)。治疗后,实验组FEV、FVC%均高于治疗前,且高于对照组,差异均具有统计学意义(P<0.05);对照组FVC%高于治疗前,差异具有统计学意义(P<0.05),FEV与治疗前对比,差异无统计学意义(P>0.05)。结论采用噻托溴铵吸入粉剂联合小剂量阿奇霉素治疗稳定期支气管扩张症效果明显,值得在临床上推广使用。Objective To investigate the effect of tiotropium bromide inhalation powder combined with low-dose azithromycin in the treatment of stable bronchiectasis. Methods A total of 40 stable bronchiectasis patients were divided by two-color ball method into control group and experimental group, with 20 cases in each group. The control group was treated with azithromycin conventionally, and the experimental group was treated tiotropium bromide inhalation powder combined with low-dose azithromycin. Observation and comparison were made on lung function index [forced expiratory volume(FEV) and percentage of forced vital capacity(FVC%)] and efficacy between the two groups. Results After treatment, the experimental group had 11 cure cases, 6 effective cases, and 3 ineffective cases, with total effective rate as 85.00%. The control group had 6 cure cases, 5 effective cases, and 9 ineffective cases, with total effective rate as 55.00%. The experimental group had higher total effective rate than the control group, and the difference was statistically significant(P<0.05). Before treatment, there was no statistically significant difference in FEV and FVC% between the two groups(P>0.05). After treatment, the experimental group had higher FEV and FVC% than those before treatment and those in the control group after treatment. Their difference was statistically significant(P<0.05). The control group had higher FVC% than thatbefore treatment, and the difference was statistically significant(P<0.05), and there was no statistically significant difference in FEV comparing with that before treatment(P>0.05). Conclusion Tiotropium bromide inhalation powder compared with low dose azithromycin shows obvious effect in the treatment of stable bronchiectasis, and it is worthy of clinical promotion and application.
