全方位优化管理模式在关节外科无痛病房康复护理中的应用

摘　　要：目的 探讨全方位优化管理模式(Overall Every Control and Clear,OEC)在关节外科无痛病房康复护理中的应用效果。方法 研究对象为本院关节外科无痛病房2014年1月~2017年8月收治的100例患者,按照计算机随机数字法随机分为2组,每组50例。对照组实施常规康复护理,观察组在康复护理中应用OEC模式,干预后比较2组患者在疼痛评分、负性情绪评分、睡眠质量评分、住院时间、康复训练依从性、生活质量评分、护理满意度等方面的差异。结果 术后12h、24h、48h,观察组的疼痛评分均低于对照组(P<0.05);2组患者护理后的负性情绪评分、睡眠质量评分均较护理前显著降低(P<0.05),而在护理后,观察组的负性情绪评分、睡眠质量评分均低于对照组(P<0.05);观察组的住院时间明显短于对照组(P<0.05);观察组的康复训练总依从率为94%,对照组为76%,组间比较差异有统计学意义(P<0.05);2组患者护理后生活质量评分较护理前增高(P<0.05),而护理后,观察组高于对照组(P<0.05);观察组的护理总满意率为96%,明显高于对照组的80%(P<0.05)。结论 在关节外科无痛病房患者的无痛康复护理中应用OEC模式,可有效减轻患者术后疼痛,缓解其负性情绪,有利于改善患者睡眠质量和生活质量,使其康复训练依从性得到提高,有利于促进其康复。Objective To explore the effect of Overall Every Control and Clear model(OEC) in rehabilitation nursing of painless joint surgery ward. Methods The subjects were 100 cases of joint surgery in our hospital painless ward from January 2014 to August 2017.They were divided into two groups by computer random number table,each group had 50 cases.The control group received routine rehabilitation nursing,while the observation group applicated OEC model in rehabilitation nursing.Pain scores,negative mood scores,sleep quality scores,duration of hospitalization,rehabilitation training compliance,quality of life score nursing,satisfaction and other aspects were compared between two groups. Results After operation 12h,24h ,48h,pain scores of the observation group were lower than that of control group(P< 0.05 );negative emotion scores sleep quality of two groups after nursing were significantly lower than that before treatment(P< 0.05 ),and after nursing,the negative emotion scores of the observation group, the sleep quality scores were lower than the control group(P< 0.05 );the hospitalization time of the observation group was significantly shorter than that of the control group(P< 0.05 );the total rehabilitation training group compliance rate of obsercation group was 94%,higher than that 76% in the control group(P< 0.05 );after nursing,life quality scores of two group was significant higher that of nursing before(P< 0.05 ),and increased in comparison between two groups after nursing,the observation group was higher than that of control group(P< 0.05 );the observation group nursing satisfaction rate was 96%,significantly higher than 80% in the control group([P< 0.05 ). Conclusion The application of OEC model in painless rehabilitation nursing care of patients in painless joint surgery ward,can effectively reduce postoperative pain,relieve the negative emotions, improve sleep quality and quality of life of patients,improve the compliance of rehabilitation training,so it is conducive to promoting their rehabilitation.
