复方丹参滴丸对2型糖尿病下肢血管病变成纤维细胞生长因子21及载脂蛋白M水平的干预作用

摘　　要：目的:研究复方丹参滴丸治疗2型糖尿病下肢血管病变的临床疗效及其对纤维细胞生长因子21(fibroblast growth factor 21,FGF21)、载脂蛋白M(Apolipoprotein M,Apo M)水平的影响。方法:选取2型糖尿病合并下肢血管病变患者120例,按照随机数字表法,1∶1将其随机分为观察组和对照组各60例。两组患者均给予基础治疗,治疗组加用复方丹参滴丸治疗,每次15粒,每日3次口服;疗程为1年。观察患者HbA1c、FPG、2hPG、TC、TG、HDL-C、LDL-C、血清FGF21、Apo M水平;测定踝/肱指数(ABI)及下肢血管动脉内径。结果:治疗前两组在HbA1c、FPG、2hPG、TC、TG、HDL-C、LDL-C、血清FGF21、Apo M水平;测定踝/肱指数(ABI)及下肢血管动脉内径两组无统计学差异(P>0.05);治疗后两组以上指标水平较治疗前改善(P<0.05),且治疗组优于对照组(P<0.05)。结论:复方丹参滴丸可以有效改善2型糖尿病下肢血管病变患者踝/肱指数(ABI)及下肢血管动脉内径,其机制可能是通过调节患者体内FGF21及ApoM水平实现的。Objective:To study the effect of compound salvia miltiorrhiza dripping pills on the treatment of lower extremity vascular lesions of type 2 diabetes and the effect on the level of fibrous growth factor 21(FGF21) and apolipoprotein M(Apo M).Methods:According to the random digital table method,120 patients with type 2 diabetes combined with lower extremity vascular disease were randomly divided into the observation group and the control group, 60 cases in each. Two groups were given the basic treatment. The treatment group was treated with Compound Danshen Dripping Pills, 15 capsules for each time, 3 times a day, and the treatment course was 1 year. The levels of HbA1 c, FPG, 2 hPG, TC, TG, HDL-C, LDL-C, serum FGF21 and Apo M were observed. Ankle/brachial index(ABI) and lower limb artery diameter were measured.Results:Before treatment, there were no statistical differences between the two groups(P>0.05) for HbA1 c, FPG, 2 hPG, TC, TG, HDL-C, LDL-C, serum FGF21, Apo M, ankle/brachial index(ABI) or the vascular artery diameter of the lower extremities. After treatment, the level of the two groups was better than that before treatment, and the treatment group’s was better than that of the control group.Conclusion:Compound Danshen Dripping Pills can effectively improve the ankle/brachial index(ABI) and the arterial diameter of the lower extremity in patients with type 2 diabetic lower extremity vascular disease. The mechanism may be realized by regulating the levels of FGF21 and ApoM in the patients’ body.
