益肾健脾活血方治疗原发性肾小球性血尿及其对免疫调节作用影响

摘　　要：目的:探讨益肾健脾活血方治疗原发性肾小球性血尿及其对免疫调节作用的影响。方法:选取我院收治入院的72例肾小球性血尿患者,随机分为治疗组和对照组,各36例。对照组给予西医常规基础治疗,治疗组在对照组的基础上予益肾健脾活血方治疗。比较两组患者临床疗效、尿蛋白量及尿红细胞计数、中医主要症状评分、血流变值及免疫功能指标。结果:治疗后,与对照组比较,治疗组临床总有效率为80.56%,明显高于对照组的66.67%,差异有统计学意义(P<0.05);经治疗,两组患者尿红细胞计数及尿蛋白量均有所降低,治疗组患者降低较对照组更显著(P<0.05);经治疗,两组患者血尿、腰膝酸软、神疲乏力、畏寒肢冷中医症状评分均有所降低(P<0.05),与对照组相比,治疗组改善更显著(P<0.05);经治疗,两组患者全血黏度、还原黏度及血浆黏度等血流变测量值均有所降低,治疗组患者降低较对照组更显著(P<0.05);经治疗,两组患者CD3^+、CD4^+比例及CD4^+/CD8^+免疫功能指标均有所升高,且与对照组相比,治疗组患者升高更显著(P<0.05)。结论:益肾健脾活血方具有辛温散寒、补肾温中、化瘀止血的功效,能有效降低患者尿蛋白及尿红细胞计数,提高临床疗效,值得临床推广。Objective:To investigate the clinical effect of Bushen Jianpi Huoxue Decotion on the treatment of primary glomerular hematuria and the effects on immune regulation. Methods:72 cases of glomerular hematuria patients admitted to our hospital were selected and randomly divided into the treatment group and the control group,36 cases each. The control group received conventional western medicine basic treatment,treatment group was given Bushen Jianpi Huoxue Decoction on the basis of the control group.The clinical efficacy,urine protein amount and urine red blood cell count,the score of main symptoms of traditional Chinese medicine and the blood rheological measurements and the Immune function index were compared between the two groups. Results:After treatment,compared with the control group,the total clinical effective rate of the experimental group was 80.56%,significantly higher than 66.67% of the control group,and the difference was statistically significant(P<0.05). After treatment,the urine erythrocyte count and urine protein amount in both groups were decreased,and the improvement in the treatment group was more significant than that in the control group(P<0.05). After treatment,the scores of hematuria,weakness of waist and knee,fatigue of spirit and cold fear of cold limbs of patients in both groups were decreased,and the improvement in the treatment group was more significant than that in the control group(P<0.05). After treatment,the whole blood viscosity,reduction viscosity,plasma viscosity and other blood rheological measurement values of the two groups of patients were decreased,and the decrease was more significant in the treatment group than in the control group(P<0.05). After treatment,the proportion of CD3+,CD4+ and CD4+/CD8+ and other immune function indicators in the two groups were significantly increased,and the increase was more significant in the treatment group than in the control group(P<0.05).Conclusion:Bushen Jianpi Huoxue Decoction has function of dispelling cold,reinforcing kidney and warmi
