自拟活血续断汤对闭合性骨折患者早期炎性因子与血流变各项指标影响  

Influence of Self-made Huoxue Xuduan Decoction in the Early Inflammatory Factors and Hemorheology Indicators for Closed Fracture Patients

摘　　要：目的:探讨自拟活血续断汤对闭合性骨折患者早期炎性因子与血流变各项指标的影响。方法:选取我院2017年1月—2018年6月间收治的95例闭合性骨折患者,按照随机数字表法将其分为对照组(n=47)与中医组(n=48),对照组给予常规闭合复位,中医组在对照组的治疗基础上采用自拟活血续断汤,对比两组服药治疗前后早期炎性因子(白细胞介入-6、白细胞介素-8和C反应蛋白)及血液流变学指标(全血黏度高低切变值和血浆黏度)变化;对比两组治疗后不同时间段的肿胀程度及疼痛评分。结果:治疗前两组炎性因子指标对比均无统计学意义(P>0.05),治疗后对照组C反应蛋白、白细胞介素-6以及白细胞介素-8等指标水平的下降程度均不及中医组,差异有统计学意义(P<0.05);治疗前两组血液流变学及自由基各指标检测结果均无统计学意义(P>0.05);治疗后中医组全血黏度高低切变值和血浆黏度较对照组明显改善,差异有统计学意义(P<0.05);治疗第1天两组肿胀程度和疼痛评分对比差异均无统计学意义(P>0.05);自治疗第7天开始至治疗28d,中医组肿胀程度及疼痛评分均明显低于对照组,差异有统计学意义(P<0.05)。治疗前两组Ca、P、ALP水平对比均无统计学意义(P>0.05);治疗后,两组Ca与ALP水平均呈现下降趋势,P呈上升趋势,组间对比显示中医组各指标明显优于对照组,差异有统计学意义(P<0.05)。结论:对闭合性骨折患者给予自拟活血续断汤口服,有助于降低减少早期炎性因子,改善血液流变学指标,缓解临床症状,对促进骨骼恢复意义重大,值得在临床推广使用。Objective:To study the influence of self-made Huoxue Xuduan Decoction in the early inflammatory factors and hemorheology indicators for closed fracture patients.Methods:95 closed fracture patients treated from January 2017 to June 2018 in our hospital were selected and randomly assigned to control group(n=47)and TCM group(n=48).The control group was treated with conventional closed reduction;on the basis of the former,the TCM group was treated with the self-made Huoxue Xuduan Decoction.The early inflammatory factors(interleukin-6,interleukin-8 and C-reactive protein)and hemorheology indicators(high and low shear values of whole blood viscosity and plasma viscosity)as well as swelling degree and pain scores were compared.Results:Before treatment,the inflammatory factors for two groups showed no statistically significant difference(P>0.05);after treatment,the values of C-reactive protein,interleukin-6 and interleukin-8 in TCM group were significantly lower than control group(P<0.05);before treatment,the hemorheology and free radical indicators for two groups showed no statistically significant difference(P>0.05);after treatment,the shear values of whole blood viscosity and plasma viscosity in TCM group were significantly better than control group(P<0.05);after 1 d of treatment,the swelling degree and pain scores for two groups showed no statistically significant difference(P>0.05);from seventh day of treatment to 28 d of treatment,the swelling degree and pain scores in TCM group were significantly lower than control group(P<0.05).Conclusion:Self-made Huoxue Xuduan Decoction can reduce the early inflammatory factors,improve the hemorheology indicators,relieve the clinical symptoms and promote the bone recovery.It is worthy of clinical promotion and application.
