摘　　要：目的探讨CT引导下经皮胃造瘘术在老年患者人群中的临床应用价值。方法回顾性分析福建省立医院2007年3月-2018年12月于住院期间行CT引导下经皮胃造瘘术82例老年患者的临床资料,评估造瘘成功率、术前术后营养情况及生活质量评分、术后并发症、术后疼痛指数情况。结果造瘘成功率为100%,老年患者术后2个月身体质量指数升高,血红蛋白增高,血清白蛋白增高,血清前白蛋白增高,C反应蛋白降低;老年患者术后2个月生活质量改善明显。结论CT引导下经皮胃造瘘术在局麻下即可进行,避免了其他胃造瘘方式定位不清晰、损伤大、操作时间长等缺点,具有成功率高、耐受性好、并发症少的优点,是一种安全、可行的肠内营养支持途径。Objective To investigate the clinical value of CT-guided percutaneous gastrostomy in elderly patients. Methods Clinical data of 82 elderly patients undergoing ct-guided percutaneous gastrostomy in Fujian provincial hospital from March 2007 to December 2018 were retrospectively analyzed to evaluate the success rate of gastrostomy,preoperative and postoperative nutrition situation and life quality score,postoperative complications and postoperative pain index. Results The success rate of fistula was 100%, and the body mass index, hemoglobin,serum albumin and serum prealbumin were increased and C-reactive protein was decreased in the elderly patients 2 months after surgery, which was statistically significant. The quality of life was also improved significantly2 months after surgery. Conclusion CT guided percutaneous gastrostomy was conducted under local anesthesia, it can be made to avoid unclear positioning, the stomach injuries, long operation time that can be made by other gastrostomy methods, because of high success rate, good tolerance,fewer complications, CT guided percutaneous gastrostomy is a safe and feasible method of enteral nutrition.
