摘 要:目的:探讨淫羊藿苷(ICA)和淫羊藿次苷Ⅱ(ICS-Ⅱ)对气管切开插管留置大鼠免疫功能与炎性反应的影响。方法:将72只SPF级雄性SD大鼠随机分成4组,正常对照组(A)、模型对照组(B)及模型治疗组(C、D),每组18只。制备大鼠气管切开插管留置模型。模型成功6 h后给予C组ICS-Ⅱ4 mg/(kg·d)、D组ICA 30 mg/(kg·d)灌胃,A、B组灌胃等量0.9%生理盐水。分别在24、72、168 h取外周血及肺泡灌洗液。应用流式细胞仪检测外周血T细胞亚群(CD4+、CD8+、CD4+/CD8+)水平,酶联免疫吸附法(ELISA)检测肺泡灌洗液sIgA、IL-6及外周血血清IL-6含量。结果:气管切开大鼠分别用ICA和ICS-Ⅱ干预后:C组CD4+T细胞数量在168 h明显高于B组(P<0.05),D组在72 h、168 h明显高于B组(均P<0.05);D组在168 h明显高于C组(P<0.05)。C组CD8+T细胞数量在72 h、168 h明显低于B组(均P<0.05);D组与B组相比无明显差异(P>0.05);D组在72 h明显高于C组(P<0.05)。C组CD4+/CD8+与B组相比无明显差异(P>0.05);D组在168 h明显高于A、B、C 3组(均P<0.05)。C、D两组外周血IL-6含量在3个时段均明显低于A、B组(均P<0.05);且D组在168 h明显低于C组(P<0.05)。C、D两组肺泡灌洗液IL-6含量在72 h、168 h明显低于B组,且呈逐渐降低趋势(均P<0.05)。C、D两组sIgA含量在72 h、168 h明显高于B组(均P<0.05);D组在24 h、168 h明显高于C组(均P<0.05)。结论:ICA和ICS-Ⅱ能够提高肺泡灌洗液sIgA含量,抑制肺泡灌洗液及外周血IL-6表达和改善外周血T细胞亚群失衡,对气管切开大鼠起到减轻炎症及提高免疫的作用,且ICA效果优于ICS-Ⅱ,尤以168 h显著。Objective: To investigate the effects of icariin(ICA)and icarisideⅡ(ICS-Ⅱ)on immune function and inflammatory response in rats with tracheotomy and intubation. Methods: 72 SPF male Sprague-Dawley rats were randomly divided into four groups:normal control group(A),model control group(B)and model treatment group(C,D),with 18 rats in each group.A rat tracheostomy tube indwelling model was prepared.After 6 hours of successful model,group C was given ICS-Ⅱ 4 mg/(kg·d)and group D was given ICA 30 mg/(kg·d).Group A and group B were intragastrically administered with 0.9% normal saline.Peripheral blood and alveolar lavage fluid were taken at 24 h,72 h,and 168 h,respectively.The levels of peripheral blood T cells(CD4 +,CD8 +,CD4 +/CD8 +)were detected by flow cytometry.The levels of sIgA,IL-6 in alveolar lavage fluid and serum IL-6 were detected by enzyme-linked immunosorbent assay(ELISA). Results: After tracheotomy in rats,ICA and ICS-Ⅱ were used respectively,CD4 +T cell amount in group C was significantly higher than group B at 168 h( P <0.05),and group D was significantly higher than group B at 72 h and 168 h(all P < 0.05).The D group was significantly higher than the C group at 168 h( P <0.05).The CD8 +T cell amount of group C was significantly lower than that of group B at 72 h and 168 h(all P <0.05).There was no significant difference between group D and group B( P >0.05).Group D was significantly higher than group C at 72 h( P <0.05).There was no significant difference between group C and group B in CD4 +/CD8 +( P >0.05).Group D was significantly higher than group A,B and C at 168 h(all P <0.05).The levels of IL-6 in peripheral blood of C and D groups were significantly lower than those of A and B groups( P <0.05).The D group was significantly lower than C group at 168 h( P <0.05).The levels of IL-6 in alveolar lavage fluid of C and D groups were significantly lower than those of group B at 72 h and 168 h,and they showed a decreasing trend(all P <0.05).The levels of sIgA in C and D groups were significantl
