桂西北地区210例不孕患者宫腔内人工授精结局分析

摘　　要：目的分析桂西北地区210例不孕患者夫精宫腔内人工授精(Intrauterine insemination,IUI)结局,探讨影响患者IUI结局的主要因素,以期为桂西北地区不孕患者IUI治疗提供参考,提高助孕成功率。方法选取2017年1月~2018年6月于我院生殖中心接受IUI治疗的不孕症夫妇210对,所有女性患者均接受IUI治疗,记录患者IUI结局,分析影响IUI妊娠率的主要因素。结果210例女性共接受242个周期治疗,共32周期成功妊娠,妊娠期间无并发症发生;经单因素分析发现,年龄、促排卵方案、子宫内膜厚度、子宫内膜类型、精液处理方法、前向运动精子数均是影响IUI妊娠率因素(P<0.05),而IUI次数、IUI时机与IUI妊娠率无相关性(P>0.05);经Logistic回归分析发现,年龄>35岁、子宫内膜厚度≤7mm或>14mm、A型子宫内膜、精液处理方法、前向精子运动数<5×106均是影响IUI妊娠率的高危因素(OR>1,P<0.05);促排卵周期与IUI周期妊娠率无明显相关性(OR<1,P>0.05)。结论IUI宫内成功妊娠率较低,女性年龄>35岁、子宫内膜厚度较薄或厚、前向精子运动数少是导致其妊娠率低的主要原因,临床中应综合分析女性患者的年龄、不孕原因及男性精子活力,为其选择合适的助孕方案以提升助孕成功率。Objective To analyze the intrauterine insemination(IUI)outcome of 210 infertility Patients in northwest Guangxi,to investigate the main influencing factors of IUI,and to Provide reference for treatment of IUI of infertility Patients in northwest Guangxi,so as to increase the success rate of assisted Pregnancy.Methods 210 infertile couPles with IUI treatment in the reProductive center from January 2017 to June 2018 were selected,and all the female Patients received IUI treatment.The IUI outcome of Patients were recorded,and the main influencing factors of IUI Pregnancy rate were analyzed.Results 210 female Patients received a total of 242 cycles of treatment,32 cycles of successful Pregnancy,without comPlications during Pregnancy;According to single-factor analysis,the age,ovulation inducing scheme,endometrial thickness,endometrium tyPes,sPerm handling methods,forward movement sPerm number were influencing factors of IUI Pregnancy rate(P<0.05),while the IUI times and IUI timing had no correlation with IUI Pregnancy rate(P>0.05);According to Logistic regression analysis,aged>35,endometrial thickness≤7mm or>14mm,tyPe A endometrium,sPerm handling methods,forward movement sPerm number<5×106 were high risk factors of influencing IUI Pregnancy rate(OR>1,P<0.05);The ovulation inducing cycle had no significant correlation with IUI Pregnancy rate(OR<1,P>0.05).Conclusion The successful intrauterine Pregnancy rate of IUI Patients is lower,aged>35,thinner or thicker endometrial thickness,less forward movement sPerm are main reasons for low Pregnancy rate.It is necessary to comPrehensively analyze the age of women,causes of infertility and sPerm motility of men,and choose the aPProPriate assisted Pregnancy Program,so as to imProve the success rate of assisted Pregnancy.
