卵巢静脉栓塞联合泡沫硬化剂治疗盆腔静脉淤血综合征的疗效分析

摘　　要：目的探讨卵巢静脉栓塞联合泡沫硬化剂治疗盆腔淤血综合征(PCS)的疗效.方法回顾性分析2010年1月到2016年1月甘肃省人民医院血管外科收治的52例PCS患者的临床资料.统计卵巢静脉栓塞的成功率和栓塞过程中的并发症及对手术的满意度;采用视觉模拟评分法(VAS)评估手术前后患者疼痛评分;对术后和随访期间疗效判定.结果手术技术成功率100%,患者满意度100%,术中及术后均无大出血的发生.术后57.7%(30/52)患者术后治愈;术后12和24个月分别有3.9%(2/52)和15.4%(2/52)例患者无效并复发.52例患者VAS评分,手术前(6.65±1.40)分高于手术后(1.85±0.31),差异有统计学意义(P<0.05).结论卵巢静脉栓塞联合泡沫硬化剂治疗PCS能明显地改善患者的临床症状,手术成功率较高和术后复发率较低.Objective To investigate the efficacy of ovarian vein embolization combined with foam sclerotherapy in the treatment of pelvic congestion syndrome(PCS). Method The clinical data of 52 patients with PCS admitted to the department of Vascular Surgery of Gansu People's Hospital from January 2010 to January 2016 were retrospectively analyzed. The success rate of ovarian vein embolization, complications during embolization and satisfaction with operation were counted. Visual analogue scale(VAS) was used to evaluate the pain score of patients before and after operation, and the curative effect was judged after operation and during follow-up. Result The success rate of surgical technique was 100%, and the satisfaction rate of patients was 100%. There was no massive hemorrhage during and after operation. 57.7%(30/52) of the patients were cured after operation;3.9%(2/52) and 15.4%(2/52) of the patients were ineffective and relapsed 12 and 24 months after operation, respectively. The VAS score of 52 patients before operation(6.65±1.40) was higher than that after operation(1.85±0.31), the difference was statistically significant(P 0.05).Conclusion Ovarian vein embolization combined with foam sclerotherapy for PCS can significantly improve the clinical symptoms of patients, with higher success rate and lower postoperative recurrence rate.
