摘 要:目的探讨彩色多普勒超声(彩超)对于下肢深静脉血栓(DVT)的诊断价值.方法选取安庆市第一人民医院2016年1月至2019年1月收治的100例下肢DVT患者彩超资料将其作为DVT组,以同期在本院接受健康体检的100例健康受检者作为健康组.比较两组静脉内径和彩色多普勒血流显像(CDFI)值;以下肢DVT诊断标准为金标准,分析彩超诊断结果与金标准间一致性及预测值;计算彩超诊断效能指标,以受试者工作曲线(ROC)下面积(AUC)分析彩超对下肢DVT的诊断价值.结果DVT组患者静脉内径均大于健康组;DVT组静脉CDFI值均低于健康组,差异有统计学意义(P<0.05).彩超诊断与金标准结果间具有高度一致性(Kappa=0.840,P<0.001);诊断符合率为92.00%、阳性预测值91.18%和阴性预测值为92.86%;彩超诊断DVT的AUC为0.920(0.831~0.967),灵敏度为93.00%,特异度为91.00%.结论彩超对下肢DVT具有较高诊断价值,且无创、重复性良好可应用于临床诊断、辅助治疗和疗效评估.Objective To explore the diagnostic value of color doppler ultrasonography in deep venous thrombosis(DVT) of lower extremities. Method The color Doppler ultrasound data of 100 patients with DVT of lower extremities admitted to Anqing First People's Hospital from January 2016 to January 2019 were selected as the DVT group, and 100 healthy subjects who underwent health examination in the same period were selected as the health group. The intravenous diameter and color doppler flow imaging(CDFI) values of the two groups were compared;the diagnostic criteria of lower limb DVT were gold standard, and the consistency and predictive value between the diagnostic results of color doppler ultrasound and gold standard were analyzed;the diagnostic efficiency index of color doppler ultrasound was calculated, and the diagnostic value of color doppler ultrasound for lower limb DVT was analyzed by the area under curve(AUC) the receiver operating curve(ROC). Result The intravenous diameter of DVT group was larger than that of healthy group, and the CDFI value of DVT group was lower than that of healthy group,the difference was significant(P<0.05). The results of color Doppler ultrasonography and gold standard were highly consistent(Kappa = 0.840, P<0.001).The diagnostic accuracy was 92.00%, positive predictive value 91.18% and negative predictive value 92.86%.The AUC of color Doppler ultrasound diagnosis of DVT was 0.920(0.831~0.967), sensitivity 93.00%, specificity91.00%. Conclusion Color doppler ultrasonography has high diagnostic value for DVT of lower extremities, and it is noninvasive and reproducible. It can be used in clinical diagnosis, adjuvant therapy and curative effect evaluation.
