单侧和双侧卵巢子宫内膜异位症抗苗勒管激素水平及与年龄变化的关系

摘　　要：目的探讨单双侧卵巢子宫内膜异位症术前抗苗勒管激素(anti-muller tube hormone,AMH)水平变化及与年龄之间的变化规律。方法选择2014年1月-2017年12月温州医科大学第二附属医院确诊的167例卵巢子宫内膜异位症患者,根据卵巢受累情况分单侧和双侧卵巢子宫内膜异位症,收集同期80例健康女性作为对照组。采用电化学发光分析仪检测血清AMH,并进行统计学分析。结果双侧卵巢子宫内膜异位症患者AMH水平明显低于单侧患者和对照;卵巢子宫内膜异位症患者及对照组AMH水平与年龄呈明显负相关(P <0. 05),其中双侧卵巢子宫内膜异位症AMH水平与年龄负相关程度明显低于单侧卵巢子宫内膜异位症与对照;年龄≤40岁双侧卵巢子宫内膜异位症患者AMH水平均显著低于单侧患者和对照组,差异均有统计学意义(P <0. 05)。结论双侧卵巢子宫内膜异位症可显著降低卵巢功能储备,为临床判断卵巢子宫内膜异位症患者手术前卵巢储备提供理论依据。Objective To investigate the changes of AMH level and age in patients with unilateral and bilateral ovarian endometriosis. Methods A total of 167 ovarian endometriosis patients diagnosed in the Second Affiliated Hospital of Wenzhou Medical University were enrolled from January 2014 to December 2017. All patients were divided into unilateral and bilateral ovarian endometriosis according to ovarian involvement. At the same time,80 healthy women were collected as healthy controls. Serum AMH was detected by electrochemiluminescence analyzer, and the results were conducted for statistical analysis.Results AMH levels in patients with bilateral ovarian endometriosis were significantly lower than those in unilateral endometriosis and healthy controls. The AMH level was negatively correlated with age in patients with ovarian endometriosis and the control group( P < 0. 05). The AMH level of bilateral ovarian endometriosis was significantly lower than that of unilateral ovarian endometriosis. Compared with the control group,AMH levels in patients with bilateral ovarian endometriosis aged ≤40 were lower than those in unilateral patients and controls,with the differences statistically significant( P < 0. 05). Conclusion Unilateral ovarian endometriosis did not significantly reduce ovarian reserve,and bilateral ovarian endometriosis significantly reduced ovarian reserve,which helps to determine ovarian reserve in patients with ovarian endometriosis before surgery.
