椎间盘镜手术治疗第五腰椎横突肥大综合征的前瞻性临床研究

摘　　要：目的探讨椎间盘镜手术治疗第五腰椎横突肥大综合征的临床疗效。方法纳入第五腰椎横突肥大综合征患者46例,随机分为保守治疗组(15例)、传统手术治疗组(15例)和椎间盘镜手术治疗组(16例)。比较三组的临床疗效以及术中术后情况。结果保守治疗组、传统手术治疗组和椎间盘镜手术组优良率分别为60.00%、80.00%、87.50%,但组间差异未达到统计学意义(P>0.05)。椎间盘镜手术组术中出血量、引流管放置天数和引流量均明显少于传统手术治疗组,住院天数短于传统手术治疗组,差异有统计学意义(t分别=14.00、11.07、12.59、6.23,P均<0.05);但两组手术时间比较,差异无统计学意义(t=1.29,P>0.05)。结论相比保守治疗和传统手术治疗,椎间盘镜手术治疗第五腰椎横突肥大综合征有一定的临床疗效优势,且创伤小、术中出血量少、术后恢复快。Objective To investigate the efficacy of microoendoscopic discectomy in the treatment of the 5 th transverse process of lumbar vertebra. Methods A total of 46 patients with the 5 th transverse process of lumbar vertebra were enrolled and divided into three groups:conservative treatment group(n=15),traditional surgical group(n=15),and microoendoscopic discectomy group(n=16).Clinical efficacy and conditions of intraoperative and postoperative were observed and compared. Results The excellent rate of conservative treatment group,traditional surgical group and microoendoscopic discectomy group were 60.00%,80.00%,87.50% respectively,the difference was not statistically significant(P>0.05).The intraoperative bleeding volume,drainage tube placement days and drainage volume in the microoendoscopic discectomy group were significantly less than those in the traditional surgical group,and the hospitalization time was shorter than that in the traditional surgical group(t=14.00,11.07,12.59,6.23,P <0.05),but there was no significant difference in the operation time between the two groups(t=1.29,P>0.05). Conclusion Compared with conservative treatment and traditional surgical treatment,microendoscopic discectomy in the treatment of the 5 th transverse process of lumbar vertebra has a certain clinical advantages of less trauma,less bleeding volume,and faster postoperative recovery.
