摘  要:目的 探讨高原地区肺动脉高压(PH)与非PH患者血清中细胞凋亡蛋白Bax、Bcl-2水平差异性。方法 以居住在平均海拔2260m的患者为研究对象,采用多普勒彩超测定肺动脉收缩压,根据有否PH分为两组,PH组123例,非PH组38例,收集临床一般资料,检测血常规、生化、心脏彩超及血清中Bax、Bcl-2水平,并进行分析。结果 PH组与非PH组的氧饱和度(SpO2)、中性粒细胞百分比、红细胞分布宽度(RDW)-SD、RDW-CV、左房前后径(LAD)、射血分数(EF)、左室短轴缩短率(LVFS)、主肺动脉内径(MPAD)、右心房左右径(RAD)、右心室左右径(RVD)、右室流出道前后径(RFAD)、总蛋白(TP)、白蛋白(Alb)、尿素氮(BUN)、总胆红素(TBIL)、直接胆红素(DBIL)、胱抑素-C差异有统计学意义(P<0.05)。PH组与非PH组血清中Bax、Bcl-2蛋白表达差异有统计学意义(P<0.05)。结论 高海拔地区的缺氧环境促进了PH的形成;血清中抑制细胞凋亡蛋白表达高水平,促进细胞凋亡蛋白表达水平较低,导致细胞增殖与凋亡失衡,参与了PH的形成。Objective To investigate the differences of apoptosis protein Bax and Bcl-2 in the serum of pulmonary hypertension (PH) and non-PH in plateau area. Methods People living in an average altitude of 2 260 m were selected as the research objects. Colour doppler ultrasonography was used to measure the pulmonary artery systolic pressure. Patients were divided into PH group (123 cases) control group (38 cases without PH). Patient's clinical general information, blood routine examination, biochemical examination, heart colour doppler ultrasonography examination were collected, Bax, Bcl-2 levels in serum were detected, above indexes were analyzed. Results There were significant differences in SpO 2, NEU%, RDW-SD, RDW-CV, LAD, EF, LVFS, MPAD, RAD, RVD, RFAD, TP, ALB, BUN, TBIL, DBIL, Cys-C between PH and control groups( P <0.05). There were significant differences in serum Bax and Bcl-2 between two groups ( P <0.05), and Bax was lower and Bcl-2 was higher in PH group than those of control group. Conclusions Hypoxia environment in high altitude area promotes the formation of PH, the high level of inhibitory apoptosis protein in serum and the lower expression of apoptosis protein lead to the imbalance of cell proliferation and apoptosis,which is involved in the formation of PH.
