皮肤镜在色素性紫癜性皮病诊断中的应用

摘　　要：目的描述色素性紫癜性皮病(PPD)的皮肤镜特征,探讨皮肤镜在PPD诊断与鉴别诊断中的价值。方法回顾性分析2016年1月2017年12月于中国中医科学院广安门医院就诊行皮肤镜检查和组织病理检查的PPD患者的皮肤镜照片,评估背景颜色、血管结构及非血管特征3方面的特征。结果共选取28例患者的69处皮肤镜照片,PPD皮肤镜下最常见的背景颜色为铜红色(59.4%,41/69),最常见的血管结构为红色点和红色小球(分别68.1%,47/69和95.7%,66/69)。最常见的非血管性结构为棕色点和棕色小球(分别60.1%,42/69)和棕色网状结构(79.7%,55/69)。结论PPD有特征性的皮肤镜表现,皮肤镜在本病的诊断和鉴别诊断中有很大的价值。Objective In order to investigate the dermoscopic findings of pigmented purpuric dermatosis (PPD),and to explore the roles of dermoscopy in the diagnosis of PPD.Methods This was a retrospective study.A total of 28 patients diagnosed histopathologically with PPD were recruited from department of dermatology,Guang'anmen hospital,China academy of Chinese medical sciences from December 2016 to October 2017.Dermoscopic features of background color,vascular morphology and non-vascular dermoscospic criteria were evaluated.Results Sixty-nine dermoscopic images of 28 patients were reviewed in this study.Coppery-red pigmentation (59.4%,41/69),red dots and globules (68.1%,47/69 and 95.7%,66/69,respectively) were most common background colors and vascular patterns in our PPD patients.Most common non-vascular dermoscopic criteria included brown dots (60.1%,42/69) and networks (79.7%,55/69).Conslusion PPD shows specific dermoscopic patterns in PPD.Dermoscopy could be useful in the diagnosis of PPD.
