肺功能低下患者电视胸腔镜下肺叶切除术后肺功能观察

摘　　要：目的探讨肺功能低下患者电视胸腔镜下行肺叶切除术后的肺功能变化情况.方法选取本院于2018年1月至2018年12月收治的60例肺功能低下患者,按随机数字表法分为两组,每组30例,对照组行传统开胸手术,观察组则在电视胸腔镜下行肺叶切除术,对比两组手术前及术后1个月时的最大分钟通气量(MVV)、用力肺活量(FVC)与肺功能第1秒用力呼气量分数(FEV1),另比较两组术前及术后3 d时的动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2).结果术后1个月时,观察组MVV、FVC、FEV1均显著高于对照组(P<0.05).术后3 d时,观察组PaCO2较对照组偏低(P<0.05),PaO2较对照组偏高(P<0.05).结论针对肺功能低下患者,行电视胸腔镜下肺叶切除术,能显著减少对肺功能的影响,临床应用价值高.Objective To investigate the changes of pulmonary function in patients with pulmonary hypofunction after video-assisted thoracoscopic lobectomy. Methods 60 patients with pulmonary hypofunction admitted to our hospital from January 2018 to December 2018 were randomly divided into two groups, 30 cases in each group. The control group underwent traditional thoracotomy, while the observation group underwent lobectomy under video-assisted thoracoscopy. The maximum minute ventilation volume (MVV), forced vital capacity (FVC) and the first second effort of pulmonary function before and after operation were compared between the two groups. The expiratory volume fraction (FEV1) was compared with the partial pressure of arterial oxygen (PaO2) and partial pressure of arterial carbon dioxide (PaCO2) before and 3 days after operation. Results MVV, FVC and FEV1 in the observation group were significantly higher than those in the control group at 1 month after operation (P<0.05). At 3 days after operation, PaCO2 in the observation group was lower than that in the control group (P<0.05), and PaO2 in the observation group was higher than that in the control group (P<0.05). Conclusion Video-assisted thoracoscopic lobectomy for patients with pulmonary hypofunction can significantly reduce the impact on pulmonary function and has high clinical value.
