成人甲状腺结节与性激素水平的相关性分析

摘　　要：目的考察甲状腺结节患者的血清性激素水平;明确恶性甲状腺结节与性激素水平的相关性。方法随机选取2017年1~12月在丰城市人民医院普外科确诊的成人甲状腺结节患者180例(男90例,女90例)。采用放射免疫法分别测定男性和女性甲状腺结节患者的卵泡刺激素(FSH)、促黄体生成素(LH)、雌二醇(E2)、催乳素(PRL)、孕酮(P)和总睾酮(TT)水平。结果男性恶性甲状腺结节患者的FSH、LH、E2、TT水平显著高于男性良性甲状腺结节患者,差异有统计学意义(P<0.05)。女性恶性甲状腺结节患者的FSH、LH、E2、PRL、P水平显著高于女性良性甲状腺结节患者,差异有统计学意义(P<0.05)。男性恶性甲状腺结节的发生与FSH、LH、E2、TT水平成正相关(P<0.01);女性恶性甲状腺结节的发生与FSH、LH、E2、P、PRL水平成正相关(P<0.05)。结论甲状腺结节的发生及分化与性激素水平存在相关性,对甲状腺结节患者进行性激素水平检查有助于甲状腺结节良恶性的诊断,值得临床推广使用。Objective To investigate the serum sex hormone levels in patients with thyroid nodules and to determine the correlation of malignant thyroid nodules and sex hormone levels.Methods From January to December 2017,180 adult patients with thyroid nodules(90 males and 90 females)were randomly selected and diagnosed in the General Surgery Department of Fengcheng People′s Hospital.The levels of follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2),prolactin(PRL),progesterone(P)and total testosterone(TT)were measured by radioimmunoassay method.Results The levels of FSH,LH,E2,TT in male patients with malignant thyroid nodules were significantly higher than those in male patients with benign thyroid nodules,and the differences were statistically significant(P<0.05).The levels of FSH,LH,E2,PRL,P in female patients with malignant thyroid nodules was significantly higher than those in female patients with benign thyroid nodules,and the differences were statistically significant(P<0.05).Malignant thyroid nodules in males were positively correlated with FSH,LH,E2 and TT levels(P<0.01);malignant thyroid nodules in females were positively correlated with FSH,LH,E2,P and PRL levels(P<0.05).Conclusion The level of sex hormone is related to the occurrence and differentiation of thyroid nodule,detection of sex hormone levels in patients with thyroid nodules is helpful to the diagnosis of benign and malignant thyroid nodules,and is worthy of clinical promotion and application.
