声带良性增生性病变患者手术前后嗓音学参数的变化情况分析

摘　　要：目的分析声带良性增生性病变患者手术前后嗓音学参数的变化情况。方法选择2016年8月~2017年10月我院就诊的53例声带息肉(声带息肉组)和47例声带小结(声带小结组)患者作为研究对象,选择同期的30例健康者为健康对照组。采用嗓音学分析软件对声带息肉组和声带小结组显微手术前后的嗓音学指标进行测定,并与健康对照组进行比较。结果声带息肉组和声带小结组患者的Jitter值、Shimmer值显著高于健康对照组,DSI值显著低于健康对照组,差异均有统计学意义(P<0.05);声带小结组患者术后第2、4周的Jitter、Shimmer、DSI值与同期健康对照组比较,差异无统计学意义(P>0.05);声带息肉组患者术后第2周的Jitter、Shimmer、DSI值与同期健康对照组比较,差异有统计学意义(P<0.05);声带息肉组患者术后第4周的Jitter、Shimmer、DSI值与同期健康对照组比较,差异无统计学意义(P>0.05)。结论声带息肉和声带小结均存在显著嗓音受损,声带息肉患者术后嗓音质量恢复到正常水平的时间长于声带小结患者,声带息肉术后创伤相比于声带小结较大。Objective To analyze the changes situation of voice parameters in patients with benign vocal cord proliferative lesions before and after surgery.Methods From August 2016 to October 2017,53 patients with vocal cord polyps(the vocal cord polyps group)and 47 patients with vocal cord nodules(the vocal cord nodules group)were selected as the study subjects,while 30 healthy persons were selected as the healthy control group during the same period.The voice parameters of patients in the vocal cord polyps group and the vocal cord nodules group before and after microsurgery were measured by voice analysis software,and the results were compared with those of healthy control group.Results The Jitter and Shimmer values of patients in the vocal cord polyps group and the vocal cord nodules group were higher than those in the healthy control group,and the DSI values in the vocal cord polyps group and the vocal cord nodules group were lower than those in the healthy control group,with significant differences(P<0.05).There was no difference in Jitter,Shimmer and DSI values at the 2nd and 4th week after operation between the vocal nodules group and the healthy control group(P>0.05).The Jitter,Shimmer and DSI values of patients in the vocal cord polyps group at the 2nd week after operation were significantly different from those in the healthy control group(P<0.05).The Jitter,Shimmer and DSI values of patients in the vocal cord polyps group at the 4th week after operation were not different from those of the healthy control group(P>0.05).Conclusion Vocal cord polyps and nodules have significant voice damage.Vocal cord polyps patients′voice quality recover to normal level after operation is longer than those with vocal cord nodules,and vocal cord polyps have greater trauma than vocal cord nodules.
