全麻插管行奥布卡因喷雾表面麻醉在支撑喉镜手术中的麻醉效果

摘　　要：目的探讨全麻插管同时行奥布卡因喷雾表面麻醉在支撑喉镜手术中的麻醉效果。方法选取2017年1月~2018年1月我院收治的60例接受支撑喉镜手术治疗患者作为研究对象,根据手术开展的先后时间分为对照组(30例)和观察组(30例)。对照组接受常规静脉全麻诱导麻醉,观察组在常规静脉全麻诱导麻醉的基础上采用盐酸奥布卡因喷雾剂于咽喉部、会厌谷和声门处开展表面麻醉。比较两组不同时点的心率(HR)、平均动脉压(MAP),比较两组丙泊酚用量、术后苏醒时间及术后躁动和喉痛的发生情况。结果观察组T1、T2、T3和T4的HR以及MAP低于对照组,差异有统计学意义(P<0.05);观察组麻醉总用药量少于对照组,术后苏醒时间短于对照组,差异有统计学意义(P<0.05);观察组躁动发生率和喉部视觉模拟量表(VAS)评分低于对照组,差异有统计学意义(P<0.05)。结论全麻插管同时行奥布卡因喷雾表面麻醉可使支撑喉镜手术患者的心率及血压稳定得到维持,减少全身麻醉用药量,缩短术后苏醒时间,同时可降低拔管过程中躁动及喉痛的发生率。Objective To investigate the anesthetic effect of general anesthesia intubation combined with Obucaine Spray in the operation of supporting laryngoscope.Methods Sixty patients received laryngoscopic surgery in our hospital from January 2017 to January 2018 were selected as the subjects of study.According to the time of operation,the patients were divided into control group(30 cases)and observation group(30 cases).The control group received general intravenous anesthesia induction,while the observation group received topical anesthesia with Obucaine Spray at pharynx,larynx,epiglottis and glottis on the basis of general intravenous anesthesia induction.Heart rate(HR)and mean arterial pressure(MAP)at different time points were compared between the two groups,and the amount of propofol,postoperative recovery time,postoperative agitation and laryngeal pain were compared between the two groups.Results HR and MAP of T1,T2,T3 and T4 in the observation group were lower than those in the control group,and the differences were statistically significance(P<0.05).The total anesthetic dosage in the observation group was less than that in the control group,and recovery time was shorter than that in the control group,and the differences were statistically significance(P<0.05).The incidence of restlessness and visual analogue scale(VAS)score in the larynx in the observation group were lower than those in the control group,and the differences were statistically significance(P<0.05).Conclusion Obucaine Spray with general anesthesia can keep the heart rate and blood pressure stable in patients undergoing laryngoscope surgery,reduce the dosage of general anesthesia,shorten the recovery time,and reduce the incidence of restlessness and larynggia during extubation.
