祛风清热饮对慢性自发性荨麻疹患者D-二聚体及抗热休克蛋白70抗体表达的影响

摘　　要：目的 观察祛风清热饮对慢性自发性荨麻疹(CSU)患者D-二聚体及抗热休克蛋白70(HSP70)抗体表达的影响。方法 将64例CSU患者按照随机数字表法分为2组。对照组32例予地氯雷他定分散片治疗;治疗组32例予祛风清热饮治疗。2组疗程均为4周。另选志愿者30人作为健康对照组。检测比较3组外周血D-二聚体、抗HSP70抗体表达水平,并比较荨麻疹活动度评分(UAS7);疗程结束后1个月统计复发率。结果 治疗前,治疗组、对照组D-二聚体、抗HSP70抗体表达水平均高于健康对照组(P<0.05)。治疗后治疗组、对照组D-二聚体、抗HSP70抗体表达水平及UAS7评分均降低(P<0.05),且治疗组均低于对照组(P<0.05)。D-二聚体、抗HSP70抗体与UAS7评分之间相关性分析结果显示,治疗组及对照组治疗前后D-二聚体表达水平与UAS7评分始终呈中等强度相关(r>0.7,P<0.05),抗HSP70抗体表达水平与UAS7评分呈弱相关(r<0.4,P<0.05)。治疗组疗程结束后1个月复发率及UAS7评分均低于对照组(P<0.05)。结论 祛风清热饮能改善CSU患者外周血D-二聚体、抗HSP70抗体表达水平及UAS7评分,且能降低复发率。D-二聚体表达水平与CSU患者临床特征呈中等强度相关性,抗HSP70抗体与患者临床特征仅呈弱相关性。Objective To observe the effect of Qufeng Qingre decoction on D-dimer and anti-heat shock protein 70 (HSP70) antibody expression in patients with chronic spontaneous urticaria (CSU). Methods 64 CSU patients were divided into two groups according to random number table method. 32 cases in control group were treated by desloratadine dispersible tablets. 32 cases in treatment group were treated by Qufeng Qingre decoction. Both groups were treated for 4 weeks. Another 30 volunteers were selected as healthy control group. The expression levels of D-dimer and anti-HSP70 antibody in peripheral blood of the three groups were detected and compared, and the activity score of urticaria (UAS7) was compared. The recurrence rate was calculated at one month after the end of the treatment. Results Before treatment, the expression levels of D-dimer and anti-HSP70 antibody in treatment group and control group were higher than those in the healthy control group ( P <0.05). After treatment, the expression levels of D-dimer, anti-HSP70 antibody and UAS7 score in the treatment group and the control group were decreased ( P <0.05), and the treatment group was lower than the control group ( P <0.05). The correlation analysis between D-dimer, anti-HSP70 antibody and UAS7 score showed that the expression level of D-dimer was always moderately correlated with UAS7 score before and after treatment in treatment group and control group ( r >0.7, P <0.05), while the expression level of anti-HSP70 antibody was weakly correlated with UAS7 score ( r <0.4, P <0.05). The recurrence rate and UAS7 score in the treatment group were lower than those in the control group at one month after the end of treatment ( P <0.05). Conclusion Qufeng Qingre decoction can improve the expression of D-dimer, anti-HSP70 antibody and UAS7 score in peripheral blood of CSU patients, and can reduce the recurrence rate. The expression level of D-dimer was moderately correlated with the clinical characteristics of CSU patients, while the anti-HSP70 antibody was only weakly
