混合痔术后肛缘水肿的中药加温湿敷中医优化护理方案疗效评价

摘　　要：目的:针对肛肠科最常见的肛缘水肿的中医护理方案进行完善和优化,评价以"中药加温湿敷"为核心的优化方案的疗效及安全性。方法:采用多中心随机对照的研究方法,选择3家单位对符合纳入标准的150例病例,随机分为试验组74例与对照组76例。试验组采用中药加温湿敷的护理操作技术,对照组采用中药坐浴的护理操作技术。比较两组患者术后肛缘水肿痊愈时间、肛缘水肿情况评分、肛门疼痛VAS评分及术后排便情况。结果:试验组患者术后7天、14天肛缘水肿评分、1~7天术后VAS疼痛评分试验组均优于对照组(P<0.05)。1~7天术后排便情况未见明显差异。结论:中药加温湿敷治疗中医优化护理方案,可有效减轻术后患者肛缘水肿程度,减轻术后肛门疼痛,是值得临床推广的临床优质护理方案。Objectives:To perfect and optimize TCM nursing scheme for the most common anal edge edema in anorectal department,and to evaluate the efficacy and safety of the optimized scheme with"warm and wet compress of traditional Chinese medicine"as the core.Methods:A multicenter randomized controlled study was conducted in which 150 patients who met the inclusion criteria were randomly divided into experimental group(74 cases)and controlled group(76 cases).The experimental group adopted the nursing operation technique of warm and wet compress of traditional Chinese medicine,while the controlled group adopted the nursing operation technique of traditional Chinese medicine hip bath.The recovery time of anal edge edema,anal edge edema score,anal pain VAS score and postoperative defecation were compared between the two groups.Results:The anal edema score(7 th day and 14 th day)and VAS pain score(1 st day to 7 th day)of the experimental group were better than those of the controlled group after operation(P<0.05).There was no significant difference in defecation after 1 st day to 7 th day.Conclusions:The optimized nursing scheme of traditional Chinese medicine plus warm and wet compress can effectively reduce the degree of edema of anal margin and anal pain of postoperative patients,which is worthy of clinical promotion.
