摘  要:目的探讨新疆油田工人油田作业职业紧张与ADD1基因、β2-AR基因交互作用对高血压的影响。方法采用整群抽样的方法,随机抽取在同一岗位工作时间1年以上共1 270名新疆油田作业工人作为研究对象,采用职业紧张量表修订版(OSI-R)进行职业紧张程度评价,PCR-RFLP技术检测ADD1基因、β2-AR基因多态性,多因子降维法进行基因交互作用分析。结果男性、汉族、工龄小于15年、倒班、钻井工种、已婚的油田工人职业紧张程度高(P<0.05)。高血压患病率随着职业紧张程度的增加有上升的趋势(χ^2=24.07,P<0.05);ADD1基因rs17833172位点GG等位基因与高血压之间存在正相关(OR=2.948,95%CI:1.2~7.1,P值<0.05),ADD1Gly460Trp位点TT等位基因与高血压之间呈负相关(OR=0.35,95%CI:0.15,0~0.78,P值<0.05),其余2个基因位点与高血压之间的关联差异无统计学意义(P值>0.05)。Gly460Trp位点、rs17833172位点、1023位点间存在交互作用,且交互模型为最优模型(P<0.05),职业紧张和ADD1基因Gly460Trp rs17833172、β2-AR基因1023、rs1042717位点间存在交互作用,且交互模型为最优模型(P<0.05)。结论职业紧张程度高的油田工人发生高血压的危险性越大,ADD1基因rs17833172位点GG等位基因与高血压的发生存在正相关,ADD1基因Gly460Trp位点TT基因与高血压之间存在负相关,ADD1 rs17833172、Gly460Trp位点与β2-AR基因rs1042717位点存在交互作用,ADD1基因、β2-AR基因的4个位点与职业紧张的交互作用对油田工人高血压的发生具有一定的调控作用。Objective To explore the effect of occupational stress and interaction between ADD1 gene and β2-AR gene on hypertension in oilfield workers in Xinjiang oilfield. Methods A stratified cluster sampling method was used to randomly select 1270 Xinjiang oilfield workers who worked in the same position for more than one year as the research objects, and the occupational stress scale revision(OSI-R) was used to evaluate the occupational stress level. PCR-RFLP Technique was used for detection of ADD1 gene and β2-AR gene polymorphism;multi-factor dimensionality reduction method was applied for gene interaction analysis. Results Male, Han nationality, working age less than 15 years, shifting, drilling work, married oilfield workers have high occupational stress(P 0.05). The prevalence of hypertension increased with the increase of occupational stress(χ2=24.07, P<0.05). There was a positive correlation between GG allele of rs1833172 locus in ADD1 gene and hypertension(OR =2.948, 95% CI: 1.2-7.1, p value 0.05), and the TT allele between ADD1 Gly460 Trp locus and hypertension was negative correlated(OR=0.35, 95% CI: 0.15, 0-0.78, p-value 0.05);the difference between the other two loci and hypertension was not statistically significant(P<0.05). There was interaction between Gly460 Trp locus, rs17833172 locus and 1023 locus, and the interaction model was the optimal model(P>0.05). There was interaction between occupational stress and ADD1 gene Gly460 Trp rs17833172,β2-AR gene 1023, rs1042717 locus. The interaction model is the optimal model(P<0.05). Conclusion The risk of hypertension in oilfield workers with high levels of occupational stress is higher. The GG allele of ADD1833172172 is positively correlated with the occurrence of hypertension. There is a negative correlation between TT1 gene and Gly460 Trp TT gene and hypertension. ADD1 rs17833172 and Gly460 Trp sites interacted with β2-AR gene rs1042717 locus.
