灯盏生脉胶囊联合氯吡格雷治疗短暂性脑缺血发作

摘　　要：目的探讨灯盏生脉胶囊联合氯吡格雷治疗短暂性脑缺血发作的临床疗效及对血清细胞因子水平的影响。方法选取2017年3月至2018年4月就诊的短暂性脑缺血发作患者130例作为研究对象,按照随机数字表法分为联合组和对照组,各65例。对照组采用氯吡格雷治疗,联合组在对照组基础上,采用灯盏生脉胶囊治疗。记录两组患者的发作次数、发作时间。检测两组的血清血管紧张素Ⅱ(Ang Ⅱ)、一氧化氮(NO)、内皮素1(ET-1)、可溶性血管细胞黏附分子1(sVCAM-1)水平。记录两组的不良反应的发生情况。结果联合组的总有效率高于对照组,差异有统计学意义(92.31% vs 80.00%,P<0.05)。治疗后,两组的发作次数、发作时间低于治疗前,且联合组的发作次数、发作时间低于对照组,差异有统计学意义(P<0.01)。治疗后,两组的Ang Ⅱ、ET-1、sVCAM-1水平低于治疗前,NO水平高于治疗前,且联合组的Ang Ⅱ、ET-1、sVCAM-1水平低于对照组,NO水平高于对照组,差异有统计学意义(P<0.01)。联合组不良反应的发生率低于对照组,差异有统计学意义(6.15% vs 18.46%,P<0.05)。结论灯盏生脉胶囊联合氯吡格雷治疗短暂性脑缺血发作的疗效确切,能显著减轻临床症状。Objective To investigate the clinical efficacy of Dengzhan Shengmai capsule combined with clopidogrel in the treatment of transient ischemic attack and its influence on serum cytokine level.Methods A total of 130 patients with transient ischemic attack were divided into control group and combined group(n=65,each).Clopidogrel was given in control group,and Dengzhan Shengmai capsule plus clopidogrel was given in combined group.Number and duration of disease episodes were recorded in two groups.The serum levels of angiotensin Ⅱ(Ang Ⅱ),nitric oxide(NO),endothelin 1(ET-1) and soluble vascular cell adhesion molecule 1(sVCAM-1) were detected,and the adverse reactions were observed in two groups.Results The total effective rate in combined group was statistically higher than that in control group(92.31% vs 80.00%,P<0.05).After treatment,the number of episodes and seizures decreased significantly compared with before treatment in both groups and were statistically lower in combined group than those in control group(P<0.01).After treatment,the serum levels of Ang Ⅱ,ET-1 and sVCAM-1 decreased,and NO level increased significantly in two groups(P<0.01);the levels of Ang Ⅱ,ET-1 and sVCAM-1 in combined group were lower than those in control group,and NO level was higher than that in control group.There were statistical differences between two groups(P<0.01).The incidence of adverse reactions in combined group was significantly lower than that in control group(6.15% vs 18.46%,P<0.05).Conclusion Dengzhan Shengmai capsule combined with clopidogrel has definite effect in treating transient ischemic attack and can obviously relieve clinical symptoms.
