MPDS在济南市院外急救分级优先调度中的应用研究

摘　　要：目的对比分析根据MPDS的急救反应编码调派的车辆与同一事件患者的病情实际所需要的急救车辆紧急程度,寻找实现院外急救分级调派的依据。方法统计分析2016年3月1日一2017年2月28日济南市急救中心调度指挥系统使用医疗优先分级调派系统(medical priority dispatch system,MPDS)的呼救事件与紧急调派系统(compvter aidos dispatch,CAD)呼救调度系统中出诊医生填写完成的同一事件所对应的患者病历11532例,以Excel形式导出医疗优先调派质量控制系统(Priority Quality Assurance,ProQA)报表和患者的电子病历数据,将MPDS的急救反应编码所对应的需要调派车辆的紧急程度与病历中患者病情危重程度实际需要的车辆调派程度相比较,分析两种派车方式的一致性。结果在预案1、13、28的事件中,MPDS急救反应级别D+E为27例,而实际病情重度的为76例,出现了急救反应不足的现象;反之,在2、4、6、10、12等预案中则出现了MPDS急救反应过度的现象。总体来说,病情重度的患者实际得到的急救车辆与依照MPDS的急救反应编码调派的急救车辆强相关(r=0.728,P=0<0.05),病情中度的患者实际得到的急救车辆与依照MPDS的急救反应编码调派的急救车辆极强相关(r=0.802,P=0<0.05),病情轻度的患者实际得到的急救车辆与依照MPDS的急救反应编码调派的急救车辆中等程度相关(r=0.577,P=0.0001<0.05)。结论在目前调度不能根据患者的病情调派急救车辆时,依据MPDS的急救反应编码级别调派急救车辆切实可行。Objective Compare first aid classification in pre-history emergency response system based on Medical priority Dispatch System (MPDS) first responder and the emergency vehicle in relation to the patient's condition.Methods A retrospective analysis was conducted on a total of 11 532 events in Jinan city where MPDS and KAD emergency scheduling system were compared from March 1,2016 to February 28,2017.The ProQA report of REPORTS in the MPDS quality control and the electronic medical record data of the patient were exported in Excel form.The urgency to dispatch the vehicle corresponding to the emergency response code of the MPDS was compared to the vehicle dispatching which was required by the critical condition of the patient in the medical record.Results The emergency vehicles actually obtained by patients with severe illness were strongly associated with emergency vehicles coded,according to the emergency response code of MPDS (r=0.728,P =0<0.05).The emergency vehicles obtained by the patient with moderate condition were highly correlated with the emergency vehicle coded according to the emergency response code of MPDS (r=0.802,P =0<0.05),while the patients with mild condition were moderately related to the emergency vehicle coded according to the emergency response code of MPDS (r=0.577,P=0.0001<0.05).Conclusion It is feasible to deploy emergency vehicles according to MPDS first aid response coding level.
