丹枝饮治疗盆腔炎后遗症寒凝血瘀疗效及对患者盆腔血流动力学影响

摘　　要：目的:探讨丹枝饮对盆腔炎后遗症寒凝血瘀证患者盆腔血流动力学影响及其临床疗效。方法:选择盆腔炎后遗症寒凝血瘀证患者153例,有77例纳入对照组,另76例纳入观察组。比较两组患者的血流动力学、临床疗效、炎性因子水平、气血指标,统计不良反应。结果:治疗后两组患者HR、MAP、CVP较治疗前升高,观察组患者HR、MAP、CVP高于对照组(P<0.05)。治疗后两组患者中医症候积分较治疗前降低,观察组患者下腹冷痛、腰骶冷痛、带下量多、月经暗红、舌质暗淡、脉沉等的积分显著低于对照组(P<0.05)。观察组患者临床总有效率显著高于对照组(P<0.05)。治疗后两组患者的TNF-α、IL-6水平低于治疗前(P<0.05),观察组患者TNF-α、IL-6水平显著低于对照组(P<0.05)。观察组患者不良反应发生率低于对照组(P>0.05)。治疗前两组患者血气指标差异比较无统计学意义(P>0.05),治疗后两组患者的pH、PaO2水平高于治疗前(P<0.05),观察组患者pH、PaO2水平高于对照组(P<0.05)。结论:丹枝饮是改善盆腔炎后遗症寒凝血瘀证患者盆腔血流动力学、气血指标,提高临床疗效的有效方法。Objective:To investigate the effect of Danzhi decoction on pelvic hemodynamics in patients with pelvic inflammatory sequelae with Hanning Xueyu syndrome.Methods:153 cases of pelvic inflammatory sequelae with cold blood stasis syndrome treated were selected and divided into two groups.77 patients in the control group 76 patients in the observation group,then the Hemodynamics,clinical efficacy,inflammatory factor levels,Qixue blood indicators,and adverse reactions were compared between groups.Results:The HR,MAP and CVP were increased after treatment,and were higher in the observation group than in the control group ( P <0.05).The TCM symptom scores had no difference between groups before treatment,which were decreased after treatment,and were lower in the observation group than in the control group ( P <0.05).The total efficacy rate was higher in the observation group than in the control group ( P <0.05).The levels of TNF-α and IL-6 were decreased after treatment,and were lower in the observation group than in the control group ( P <0.05).pH and PaO2 levels had no difference between groups before treatment,which were increased after treatment,and were higher in the observation group than in the control group ( P <0.05).Conclusion:The application of Danzhi decoction can effectively ameliorate the pelvic hemodynamics and Qi and blood indicators for pelvic inflammatory sequelae patients with Hanning Xueyu syndrome,which is recommended.
