完壁式乳突根治术治疗胆脂瘤型中耳炎的疗效分析

摘　　要：目的探讨完壁式乳突根治术并鼓室成形术治疗胆脂瘤型中耳炎的效果.方法回顾性分析郑州大学第一附属医院2015年6月至2017年6月收治的胆脂瘤型中耳炎患者322例(均为单侧)的临床资料.依据治疗方案将其分为对照组和观察组,每组161例.对照组行开放式乳突根治+鼓室成形术治疗,观察组行完壁式乳突根治+鼓室成形术治疗,术后随访1年,观察两组的术后听力恢复、干耳情况及并发症和复发率.结果术后随访1年发现,观察组干耳152例(94.41%),总有效率为97.52%,均高于对照组,差异均有统计学意义(P均<0.05).两组疾病复发率比较差异未见统计学意义(P>0.05).结论完壁式乳突根治术并鼓室成形术在治疗胆脂瘤型中耳炎可明显改善术后患者的临床疗效,提高患者生活质量,条件允许可优先选择此术.Objective To investigate the curative effects of intact canal wall mastoidectomy on patients with middle ear cholesteatoma.Methods The clinical data of 322 cases(all unilateral)of cholesteatoma otitis media admitted to the First Affiliated Hospital of Zhengzhou University from June 2015 to June 2017 were retrospectively analyzed.They were randomly divided into control group and observation group,with 161 cases in each group.The observation group usederwent intact canal wall mastoidectomy and tympanoplasty for treatment.The control group received open tympanoolasty for treatment.The two methods were evaluated based on postoperative efficacy.Results After 1 year of follow-up,152 dry ears in the observation group(94.41%),the total effective rate was 97.52%,which were significantly higher than those of the control group,the differences were significant(P all<0.05).There was no significant difference in recurrence rate between the two groups(P>0.05).Conclusions Complete mastoidectomy combined with tympanoplasty can significantly improve the clinical efficacy in the treatment of cholesteatoma otitis media,improve the quality of life of patients.This procedure has the priority,as long as conditions allow.
