VAC技术联合扩创植皮术治疗痛风石破溃的临床效果评价

摘　　要：目的:探讨真空辅助闭合(VAC)技术联合扩创植皮术治疗痛风石破溃的效果及对瘢痕情况的影响。方法:选择2016年1月至2018年1月我院收治的80例痛风石破溃患者,按照随机数字表法将患者分为观察组及对照组,各40例。观察组患者采用VAC技术联合扩创植皮术,对照组患者采用常规换药治疗。比较两组之间创面愈合率、临床指标、并发症及瘢痕的发生情况的差异。结果:伤口处理后7、14 d观察组患者的创面愈合率均大于对照组(P <0. 05);观察组患者的住院、感染控制、创面恢复时间均短于对照组,数字评分法疼痛评分均低于对照组;观察组患者皮瓣腔隙、皮肤浸渍出现几率均明显低于对照组;观察组患者的轻度瘢痕发生率明显高于对照组,差异均有统计学意义(均P <0. 05)。结论:VAC技术联合扩创植皮术治疗痛风石破溃效果良好,明显降低了患者感染风险,缩短了创面痊愈时间,美学指标优良,值得在临床工作中进行推广。Objective: To analyze effect of vacuum assisted closure( VAC) technique combined with debridement and skin grafting in the treatment of gouty stone rupture. Methods: Eighty patients with gouty stone rupture in our hospital from January 2016 to January 2018 were selected and divided into the observation group and the control group according to random number table method,40 cases in each group. Patients in the observation group were treated with VAC combined with debridement and skin grafting,while patients in the control group were treated with routine dressing change. The differences of wound healing rate,clinical index,complications and scar condition between the two groups were comparied. Results: After 7 and 14 days of wound treatment,the wound healing rate of the observation group was higher than those of the control group( all P < 0. 05);the hospitalization,infection control,wound recovery time of the observation group were shorter than those of the control group,and the pain score of numerical rating scale was lower than that of the control group;the occurrence rate of flaps lacunae and skin impregnation in the observation group was significantly lower than those of the control group. The incidence of mild scar of the observation group was significantly higher than that of the control group( P < 0. 05).Conclusion: VAC technique combined with debridement and skin grafting in the treatment of gouty stone repture has a good therapeutic effect. It could reduce the risk of infection significantly,shortens recovery time and aesthetic indicators is good. It is worth promoting.
