摘　　要：目的:探讨面动脉-鼻外侧动脉-内眦动脉皮瓣在鼻部基底细胞癌切除后创面修复中的应用。方法:2015年1月-2018年12月,收治鼻部基底细胞癌患者12例,肿瘤面积0.8cm×1.5cm^2.0cm×2.5cm,术中扩大切除边缘3~5mm,快速病理证实切缘及基底阴性,设计以面动脉-鼻外侧动脉-内眦动脉皮瓣修复创面,切取皮瓣面积1.2cm×3.2cm^2.6cm×4.8cm,供区游离后拉拢缝合。结果:所有皮瓣均成活,供、受区切口愈合良好,未出现面瘫、提上唇肌功能受损、术区皮肤麻木、口角畸形等并发症。经1~24个月随访,皮瓣色泽、质地、外形较好,肿瘤无复发。结论:面动脉-鼻外侧动脉-内眦动脉皮瓣设计灵活,转移方便,供瓣面积适中,血供可靠,色泽质地与受区接近,是修复鼻部基底细胞癌切除术后缺损的较好方法。Objective To evaluate the effect of facial artery-lateral nasal artery-inner canthus artery flap in repairing the wound after resection of nasal basal cell carcinoma. Methods We treated 12 cases of nasal basal cell carcinoma from January 2015 to December 2018 and the tumor areas ranged from 0.8 cm×1.5 cm to 2.0 cm×2.5 cm. Intraoperatively, the excision margin was enlarged by 5 mm and confirmed of negative incisional margin and basement by rapid pathology. Afterwards, we designed facial artery-lateral nasal artery-inner canthal artery flap varied from 1.2 cm×3.2 cm to 2.6 cm×4.8 cm to repair the wound. Donor area was separated and sutured directly. Results All flaps survived and the incisions in the donor and recipient area healed well. Furthermore, there were no complications such as facial paralysis, impaired function of the levator labrum, numbness in the operative area, or oral horn deformity. After following up for 1-24 months, the flap showed great superiority in color, texture and appearance with no tumor recurrence. Conclusion Facial artery-lateral nasal artery-inner canthus artery flap has the advantages of flexible design, convenient transfer, moderate area, reliable blood supply, similar color and texture to the recipient area. It’s an alternative treatment in repairing nasal defect after tumor resection.
