摘 要:目的探讨速度向量成像(VVI)技术评估不同程度代谢综合征(MS)患者左室舒张功能的改变,为临床早期干预治疗提供依据。方法选取60例MS患者,根据血糖、血压升高的不同程度分为MSⅠ组和MSⅡ组;另选同期健康体检者30例作为对照组。常规测量室间隔舒张末厚度(IVSd)、左室后壁舒张末厚度(LVPWd)、左室射血分数(LVEF)、二尖瓣口舒张早晚期血流峰值速度(E、A)、二尖瓣环舒张早期运动速度(e’),计算E/A和E/e’;VVI技术测量左室各节段舒张早期峰值速度(Ve)及峰值应变率(SRe),计算E/Ve;比较三组各参数变化情况。结果三组间LVEF比较差异无统计学意义。与对照组比较,MSⅠ组IVSd和LVPWd明显增厚,差异均有统计学意义(均P<0.01);MSⅡ组IVSd和LVPWd与对照组比较差异无统计学意义。MSⅠ组和MSⅡ组E/A值均显著低于对照组,E/e’均显著高于对照组;MSⅠ组E/e’值亦显著高于MSⅡ组,差异均有统计学意义(均P<0.01)。MSⅠ组左室各节段Ve均显著低于对照组,MSⅡ组Ve除室间隔心尖段外,其余均显著低于对照组,差异均有统计学意义(均P<0.01);MSⅠ组SRe均低于对照组,MSⅡ组SRe除了后间隔中间段、下壁中间段及前壁中间段外,其余均低于对照组,差异均有统计学意义(均P<0.01);MSⅠ组及MSⅡ组E/Ve值分别为29.20±4.08、23.58±3.02,均显著高于对照组(15.98±3.19),且MSⅠ组显著高于MSⅡ组,差异均有统计学意义(均P<0.01)。结论 VVI技术可准确敏感地评价MS患者左室舒张功能的改变。Objective To explore the changes of left ventricular diastolic function in patients with varying degrees of metabolic syndrome(MS) by velocity vector imaging(VVI),which could provide basis for early clinical intervention.Methods Sixty patients with MS were selected,according to the different degrees of blood sugar and blood pressure,they were divided into MSⅠgroup and MSⅡgroup. Another 30 healthy people were selected in the same period as control group. Routine ultrasound measurements of interventricular septal of diastole(IVSd),left ventricular posterior wall of diastole(LVPWd),left ventricular ejection fraction(LVEF),velocity of mitral flow of diastole(E,A),velocity of mitral annulus septal of early diastole(e’),E/A and E/e’were measured.VVI was used to measure the peak velocity(Ve)and peak strain rate(SRe)of left ventricular segments of early diastole,the changes of parameters among three groups were compared.Results There was no significant difference in LVEF among three groups.Compared with the control group,IVSd and LVPWd in MSⅠ group were significantly thicker(both P<0.01),while the IVSd and LVPWd in MSⅡ group was no significant difference.E/A in MSⅠ and MSⅡ groups were significantly lower than that in control group,E/e’were significantly higher than that in control group,and the E/e’in MSⅠ group was also significantly higher than that in MSⅡgroup,the differences were statistically significant(all P<0.01).The Ve values in each segment of left ventricular in MSⅠgroup were significantly lower than those in control group,except for the negative of ventricular septal apical segment,the Ve values in MS Ⅱ group were all significantly lower than those in control group,the differences were statistically significant(all P<0.01).The SRe in MS Ⅰ group was significantly lower than that in control group,except for the negative of the middle segment of the posterior septum,the middle segment of the inferior wall and the middle segment of the anterior wall,the SRe in MS Ⅱ group was lower t
