摘 要:目的探讨平均淋巴细胞计数(ALY)在系统性红斑狼疮(SLE)合并肺部感染患者短期预后中的临床价值。方法选取2013年1月至2017年12月住院的SLE合并肺部感染患者46例作为研究对象(感染组),同期选取无感染SLE患者46例作为对照组。采用受试者工作曲线(ROC曲线)评价入院24h内淋巴细胞(LY)及ALY在SLE合并肺部感染患者短期预后(30d)中的价值。结果感染组患者外周血中LY、T细胞及CD4+T细胞水平均显著低于非感染组,差异均有统计学意义(P<0.05)。感染组46例患者30d内9例发生死亡。死亡组LY及ALY水平均显著低于存活组,差异均有统计学意义(P<0.05)。LY及ALY判断预后的ROC曲线下面积分别为0.763±0.074及0.895±0.054。ALY最佳临界值为0.59×109L-1。结论SLE合并肺部感染患者淋巴细胞、T细胞、CD4+T细胞均显著降低。ObjectiveTo investigate the clinical value of mean lymphocyte count(ALY)in the short-term prognosis of patients with systemic lupus erythematosus(SLE)complicated with pulmonary infection.MethodsForty-six patients with SLE and pulmonary infection who were hospitalized from January 2013 to December 2017 were enrolled as subjects(infected group).Forty-six patients with uninfected SLE were selected as the control group.The receiver operating curve(ROC curve)was used to evaluate the value of lymphocyte(LY)and ALY in the short-term prognosis(30 d)of patients with SLE complicated with pulmonary infection within 24 h after admission.ResultsThe levels of LY,T cells and CD4+T cells in the peripheral blood of the infected group were significantly lower than those in the non-infected group(P<0.05).Forty-six patients in the infected group died within 9 days within 30 days.The LY and ALY levels in the death group were significantly lower than those in the survival group,and the difference was statistically significant(P<0.05).The area under the ROC curve for prognosis of LY and ALY was 0.763±0.074 and 0.895±0.054,respectively.The optimal cutoff value of ALY was 0.59×10 9 L-1.ConclusionLymphocytes,T cells,and CD4+T cells are significantly decreased in patients with SLE complicated with pulmonary infection.
