摘 要:目的:探讨病毒性脑炎患儿淋巴细胞亚群及脑脊液炎症因子TNF-α、IL-2、IL-6、IL-10在其发病过程中的变化规律及其临床意义。方法:选取病毒性脑炎患儿128例,同期在我院儿外科择期手术无中枢神经系统器质性病变的腰椎间隙麻醉患儿60例作为对照组,采用流式细胞技术检测其淋巴细胞亚群的表达情况,采用双抗体夹心(ELISA)法检测患儿脑脊液中TNF-α、IL-2、IL-6、IL-10浓度水平。结果:CD3^+、CD4^+、CD8^+、CD19^+、CD16^+CD56^+、CD4^+/CD8^+、CD4^+CD25^+细胞水平在轻型组、重型组和对照组之间比较,差异均有统计学意义(P<0.05),CD3^+、CD8^+、CD19^+、CD4^+/CD8^+细胞水平在治疗前后比较差异有统计学意义(P<0.05),CD4^+、CD16^+CD56^+、CD4^+CD25^+治疗前后比较差异无统计学意义(P>0.05)。脑脊液中TNF-α、IL-2、IL-6、IL-10水平重型组高于轻型组和对照组(P<0.05),轻型组高于对照组(P<0.05),治疗前高于治疗后(P<0.05)。结论:淋巴细胞的功能紊乱可能与儿童病毒性脑炎的发病机制有关;脑脊液TNF-α、IL-2、IL-6和IL-10也参与了病毒性脑炎患儿抗病毒免疫和病理过程。Objective: To observe the changes and clinical significance of peripheral blood lymphocyte subset and the cerebrospinal fluid level of tumor necrosis factor-α(TNF-α),interleukin-2(IL-2),interleukin-6(IL-6),interleukin-10(IL-10) in children with viral encephalitis. Methods: 128 cases of viral encephalitis children were selected as the research,and 60 children with undergoing anesthetized operation without organic injury in central nervour system were selected as the control group at the same period.Lymphocyte subsets were measured by flow cytometry;TNF-α,IL-2,IL-6,IL-10 of cerebrospinal fluid were detected by enzyme linked immunosorbent assay(ELISA). Results: There was significant difference in the percentage of CD3^+,CD4 ^+,CD8^+,CD19 ^+,CD16^+CD56 ^+,CD4 ^+/CD8^+,CD4 ^+CD25 ^+ among the severe group,the light group and the control group( P <0.05).There was significant difference of CD3^+,CD8 ^+,CD19^+,CD4^+/CD8 ^+ between before treatment and after treatment group( P <0.05).There was no significant difference of CD4 ^+,CD16 ^+CD56^+,CD4 ^+CD25 ^+ on the two groups.The cerebrospinal fluid level of TNF-α,IL-2,IL-6,IL-10 with the patients in severe group were significantly higher than those in the light group and the control group( P <0.05);those of the light group were significantly higher than those in the control group;those of before treatment were significantly higher than those of after treatment. Conclusion: Peripheral blood lymphocyte dysfunction may be related to the pathogenesis of children with viral encephalitis;TNF-α,IL-2,IL-6,IL-10 not only have anti-viral effect,but also play a role in pathogenesis of encephalitis in children.
