老年高血压患者脑血流动力学参数、血液指标变化以及脑白质疏松发生情况研究

摘　　要：目的研究老年高血压患者的脑血流动力学参数、血清UA、血清CK、Cys-C、APN等指标较正常人的变化及其脑白质疏松发生情况。方法选取2016年3月至2018年6月在我院进行诊治的69例老年高血压患者为研究组,并取同期在我院进行健康体检的72例血压正常人员为对照组,检测并比较两组人员的脑血流动力学参数Vm、Vs、Vd和PI,以及血UA、血清CK、Cys-C、APN等指标,并对比两组人员脑白质疏松发生情况。结果与对照组相比,研究组患者的脑血流动力学参数Vm、Vs、Vd以及PI明显较高,比较结果存在统计学差异(P<0.05);研究组患者的脑白质疏松症发生情况明显更严重,比较结果存在统计学差异(P<0.05);研究组患者血UA、血清CK和Cys-C水平均明显高于对照组,而血清APN水平明显低于对照组,比较结果存在统计学差异(P<0.05)。结论老年高血压患者脑血流速度更快,脑白质疏松症的发生率更高、病情更加严重,血UA、血清CK和Cys-C水平明显升高,而血清APN水平明显降低。监测这些指标对于判断患者病情以及预后具有重要的参考价值。Objective To explore the variation of cerebral hemodynamic parameters,the levels,of serum uric acid(UA),serum creatine kinase(CK),cystatin C(Cys-C),adiponectin(APN) and the occurrence of leukoaraiosis in the elderly with hypertension. Methods 69 elderly with hypertension during the period from March 2016 to June 2018 were selected to be observation group while 72 norm of physical examination during the same period were selected to be control group;a comparative study was made to cerebral hemodynamic parameters(Vm,Vs,Vd and PI),serum UA,serum CK,Cys-C,APN and to the occurrence of leukoaraiosis between the 2 groups. Results The hemodynamic parameters(Vm,Vs,Vd and PI) and the incidence rate of leukoaraiosis in observation group were significantly higher than those in control group(P <0. 05);the levels of serum UA,serum CK and Cys-C levels were higher while the level of APN was lower in observation group than those in control group,the difference between the 2 groups was of statistical significance(P<0. 05). Conclusions The elderly with hypertension are of faster blood flow velocity,usually with a higher occurrence and a greater severity of leukoaraiosis,the levels of serum UA,serum CK and Cys-C increase while the level of APN decreases;to monitor these indices is of important reference for evaluating the condition and prognosis of the elderly with hypertension.
