冠心病患者12导联动态心电图检查的应用探析

摘　　要：目的探析12导联动态心电图检查冠心病的应用价值。方法方便选取该院2016年6月—2017年10月疑似诊断为冠心病的76例患者,均进行常规心电图及12导联动态心电图检查,同时施行冠脉造影检查,并以此为金标准,比较常规心电图、12导联动态心电图检查冠心病的临床情况,进一步分析12导联动态心电图检查冠心病的价值。结果以冠脉造影为金标准,常规心电图、12导联动态心电图检查冠心病的敏感度、特异度、准确度为50.00%和86.36%、70.00%和90.00%、52.63%和86.84%,其中敏感度、准确度的对比差异有统计学意义(P<0.05)。常规心电图、12导联动态心电图对多支病变的检出率为100.00%和95.45%,均高于单支病变的25.00%和75.00%(χ^2=33.00、4.13,P<0.05),12导联动态心电图对单支病变的检出率为75.00%,高于常规心电图的25.00%(χ^2=22.00,P<0.05)。结论12导联动态心电图检查冠心病的敏感度、准确度高,用于冠心病的临床诊断具有一定价值。Objective To investigate the application value of 12-lead dynamic electrocardiogram in the diagnosis of coronary heart disease.Methods 76 patients with suspected coronary heart disease from June 2016 to October 2017 were conveniently selected in our hospital.All patients underwent routine electrocardiogram and 12-lead dynamic electrocardiogram coronary angiography was performed at the same time.The clinical situation of coronary heart disease was analyzed by conventional electrocardiogram and 12-lead dynamic electrocardiogram,and the value of 12-lead dynamic electrocardiogram for coronary heart disease was further analyzed.Results The coronary angiography was the gold standard.The sensitivity,specificity and accuracy of conventional electrocardiogram and 12-lead dynamic electrocardiogram for coronary heart disease were 50.00%and 86.36%,70.00%and 90.00%,52.63%and 86.84%.There was a statistically significant difference in the degree of accuracy and accuracy(P<0.05).The detection rate of conventional electrocardiogram and 12-lead dynamic electrocardiogram for multivessel disease was 100.00%and 95.45%,which were higher than 25.00%and 75.00%of single-vessel disease(χ^2=33.00,4.13,P<0.05),12-lead detection rate of single-vessel disease by dynamic electrocardiogram was 75.00%,which was higher than that of conventional electrocardiogram 25.00%(χ^2=22.00,P<0.05).Conclusion The 12-lead dynamic electrocardiogram has high sensitivity and accuracy for coronary heart disease,and it has certain value for clinical diagnosis of coronary heart disease.
