双侧髂静脉超声检查在髂静脉压迫综合征中的应用价值

摘　　要：目的讨论双侧髂静脉超声检查在髂静脉压迫综合征中的应用价值。方法回顾性分析2018年1—12月于该院162例行顺行静脉造影患者的临床资料,所有患者临床资料齐全完整,且所有患者均在顺行静脉造影进行前1周内接受双侧髂静脉超声检查;以顺行深静脉造影检查结果为金标准,讨论双侧髂静脉超声检查在髂静脉压迫综合征检查中的诊断效能。结果经顺行静脉造影检查,162例受检患者中,有41例诊断为髂静脉压迫综合征,121例为髂静脉压迫综合征阴性;双侧髂静脉超声检查的灵敏度为80.49%(33/41),特异度为97.52%(118/121)、诊断准确率为93.21%(151/162)、阳性预测值为91.67%(33/36)、阴性预测值为93.65%(118/126)。与金标准进行一致性检验,Kappa值为0.813,一致性较高。结论双侧髂静脉超声在髂静脉压迫综合征中的诊断灵敏度、特异度均较高,诊断准确率较高,与顺行深静脉造影检查结果的一致性较高,临床应用价值高。Objective To evaluate the value of bilateral iliac vein ultrasonography in the diagnosis of iliac vein compression syndrome.Methods The clinical data of 162 patients undergoing antegrade venography in our hospital from January to December 2018 were retrospectively analyzed.The clinical data of all patients were complete and all patients were accepted within 1 week before antegrade venography.Ultrasound examination of bilateral iliac veins;the results of antegrade deep venography were used as the gold standard,and the diagnostic efficacy of bilateral iliac vein ultrasonography in the examination of iliac vein compression syndrome was discussed.Results By antegrade venography,41 cases of the 162 patients were diagnosed with iliac vein compression syndrome,121 cases were negative for iliac vein compression syndrome,and the sensitivity of bilateral iliac vein ultrasound was 80.49%(33/41),the specificity was 97.52%(118/121),the diagnostic accuracy was 93.21%(151/162),the positive predictive value was 91.67%(33/36),and the negative predictive value was 93.65%(118/126).Consistency test with the gold standard,Kappa value of 0.813,high consistency.Conclusion Bilateral iliac vein ultrasound has higher diagnostic sensitivity and specificity in iliac vein compression syndrome,and has higher diagnostic accuracy.It has higher consistency with antegrade deep vein angiography results and higher clinical application value.
