摘 要:目的探讨血细胞分析仪及血细胞形态学在筛查血液系统疾病中的应用效果。方法选取2017年11月~2018年11月深圳市龙岗中心医院收治的146例疑似血常规异常患者作为研究对象,分别进行血细胞分析仪检查以及血细胞形态学检查。比较两种检查方法的血常规形态学检出率、白细胞分类各水平及血液系统疾病检出率。结果两种检查方法中的白细胞减少、核左移及红细胞异常检出率比较,差异无统计学意义(P>0.05);血细胞分析仪检查中的幼稚红细胞、异常血小板及异型淋巴细胞检出率显著高于血细胞形态学检查,差异均有统计学意义(P<0.05)。两种检查方法中的单核细胞(MO)、嗜中性粒细胞(NE)、淋巴细胞(LY)、嗜酸性粒细胞(EO)水平比较,差异无统计学意义(P>0.05);血细胞分析仪检查中的嗜碱性粒细胞(BA)水平显著高于血细胞形态学检查,差异有统计学意义(P<0.05)。两种检查方法中的急性非淋巴细胞白血病(ANLL)和地中海贫血(TH)检出率比较,差异无统计学意义(P>0.05);血细胞分析仪检查中的传染性单核细胞增多症(IM)、急性淋巴细胞白血病(ALL)、慢性髓性白血病(CML)、多发性骨髓瘤(MM)及总阳性检出率显著高于血细胞形态学检查,差异均有统计学意义(P<0.05)。结论血细胞分及总阳性析仪在血液系统常规检查、白细胞分类及血液系统疾病中具有较好的应用效果,值得临床应用推广。Objective To explore the application effect of blood cell analyzer and blood cell morphology examination in screening diseases of the blood system. Methods A total of 146 patients suspected with abnormal blood routine admitted to Longgang Central Hospital of Shenzhen City from November 2017 to November 2018 were selected as the subjects of study, blood cell analyzer and blood cell morphology were examined respectively. The detection rates of blood routine morphology, leucocyte classification and blood system diseases were compared between the two methods. Results There were no significant differences in the detection rates of leukopenia, left nuclear shift and erythrocyte abnormalities between the two methods (P>0.05). The detection rate of immature erythrocyte, abnormal platelet and abnormal lymphocyte in blood cell analyzer were significantly higher than those in blood cell morphology, the differences were statistically significant (P<0.05). There were no significant differences in the levels of monocyte (MO), neutrophil (NE), lymphocyte (LY) and eosinophil (EO) between the two methods (P>0.05). The basophilic granulocyte (BA) level in blood cell analyzer was significantly higher than that in blood cell morphology, the difference was statistically significant (P<0.05). There were no significant differences in the detection rates of acute non-lymphocytic leukemia (ANLL) and thalassemia (TH) between the two methods (P>0.05). The detection rates of infectious mononucleosis (IM), acute lymphocyte leukocyte (ALL), chronic myeloid leukemia (CML), multiple myeloma (MM) and total positive detection rate in blood cell analyzer were significantly higher than those in blood cell morphology, the differences were statistically significant (P<0.05). Conclusion Hematology analyzer has good application effect in routine examination of blood system, classification of white blood cells and diseases of blood system. It is worth popularizing in clinical application.
