骨水泥在椎体骨折线内弥散情况对椎体成形术治疗效果的影响

摘　　要：目的:研究骨水泥在椎体骨折线内弥散情况对椎体成形术的疗效影响。方法:选择笔者所在医院2015年9月-2017年9月实施椎体成形术治疗的115例患者进行回顾性分析,并根据骨水泥在椎体骨折线内弥散情况分为弥散良好组72例和弥散不良组43例。比较两组患者的治疗效果。结果:两组患者术后4 d、术后1年VAS、ODI评分均优于术前(P<0.05),且术后4 d弥散良好组VAS、ODI评分均优于弥散不良组(P<0.05)。两组患者术后4 d、术后1年伤椎矢状面后凸Cobb角均优于术前,且弥散良好组均优于弥散不良组(P<0.05)。两组患者骨折再发生率、骨水泥渗漏率比较,差异无统计学意义(P>0.05),弥散良好组椎体塌陷率低于弥散不良组(P<0.05)。结论:椎体成形术患者骨水泥在椎体骨折线内弥散情况良好,可改善术后疼痛,减少椎体塌陷率,值得推广。Objective:To study the influence of bone cement diffusion in vertebral fracture line on the therapeutic effect of vertebroplasty.Method:A total of 115 patients who underwent vertebroplasty in our hospital from September 2015 to September 2017 were retrospectively analyzed.According to the dispersion of bone cement in vertebral fracture line,72 patients were divided into good dispersion group and 43 patients into poor dispersion group.The therapeutic effects of the two groups were compared.Result:The VAS and ODI scores of the two groups at 4 days after operation and 1 year after operation were better than those before operation(P<0.05),and the VAS and ODI scores of the good diffusion group at 4 days after operation were better than those of the poor diffusion group(P<0.05). Cobb angle of sagittal kyphosis of injured vertebrae in both groups at 4 days after operation and 1 year after operation were better than that before operation,and good diffusion group was better than poor diffusion group(P<0.05).There were no significant differences in fracture recurrence rate and cement leakage rate between the two groups(P>0.05).The vertebral collapse rate of the good diffusion group was lower than that of the poor diffusion group(P<0.05).Conclusion:Bone cement in patients with vertebroplasty is well dispersed in the fracture line of vertebral body,which can improve the pain after operation and reduce the collapse rate of vertebral body.It is worth popularizing.
