摘  要:目的探讨纤维支气管镜下吸痰治疗联合无创正压通气(NIPPV)对肺部感染患者肺功能、呼吸指标及炎症因子的影响。方法将120例肺部感染患者按治疗方式分为对照组和观察组,各60例。对照组行常规治疗+NIPPV,观察组在对照组基础上行纤维支气管镜下吸痰治疗,对比两组治疗效果。结果观察组治疗总有效率为96.67%,高于对照组的85.00%(P<0.05)。治疗后,两组患者的Cdyn、TLC、OI、WOB、IL-8、TNF-α水平及CPIS评分均优于治疗前,且观察组优于对照组(P<0.05)。结论纤维支气管镜下吸痰治疗联合NIPPV对肺部感染的疗效显著,能改善肺呼吸功能,减少炎症反应和感染,值得临床应用。Objective To investigate the effect of sputum aspiration under fiberoptic bronchoscope combined with noninvasive positive pressure ventilation(NIPPV) on pulmonary function, respiratory indexes and inflammatory factors in patients with pulmonary infection. Methods One hundred and twenty patients with pulmonary infection were divided into control group and observation group according to the treatment method, with 60 cases in each group. The control group was treated with routine therapy +NIPPV, while the observation group was treated with sputum aspiration under fiberoptic bronchoscope on the basis of the control group. The treatment effects of the two groups were compared. Results The total effective rate of treatment in the observation group was 96.67%, which was higher than 85.00% in the control group(P<0.05). After treatment, the Cdyn, TLC, OI, WOB, levels of IL-8, TNF-α and CPIS score in the two groups were better than those before treatment, and those in the observation group were better than the control group(P<0.05). Conclusion Sputum aspiration under fiberoptic bronchoscope combined with NIPPV has a significant effect on pulmonary infection, it can improve pulmonary respiratory function, reduce inflammation and infection, which is worthy of clinical application.
