注射用美罗培南治疗尿脓毒血症31例诊治分析

摘　　要：目的了解尿脓毒血症的临床和实验室特征,评价注射用美罗培南治疗尿脓毒血症的临床效果及安全性。方法回顾性分析2016年1月至2019年3月在我科接受注射用美罗培南治疗的31例尿脓毒血症患者的临床资料,分析患者的一般资料、用药剂量、术式、临床疗效、临床指标水平及不良反应发生情况。结果经注射用美罗培南治疗后,患者的临床治愈率为90.32%(28/31),细菌清除率为92.86%(13/14)。治疗后,尿脓毒血症患者的WBC、SCr、BUN、PCT水平及体温均明显低于治疗前(P<0.05)。治疗后,4例患者血ALT水平升高,其中3例升高值大于治疗前水平3倍,但31例患者治疗前、后ALT水平比较,差异无统计学意义(P>0.05)。患者在治疗过程中不良反应少。结论注射用美罗培南在临床上早期经验性治疗尿源性脓毒血症的效果确切,细菌清除率和临床治愈率高,且不良反应少,临床应用价值较高。Objective To understand the clinical and laboratory characteristics of urinary sepsis, and to evaluate the clinical effect and safety of meropenem for injection in the treatment of urinary sepsis. Methods The clinical data of 31 cases of urinary sepsis treated with meropenem for injection in our department from January 2016 to March 2019 were retrospectively analyzed. The general data, dosage, operation method, clinical efficacy, levels of clinical indicators and adverse reactions were analyzed. Results After treatment with meropenem for injection, the clinical cure rate was 90.32%(28/31), and the bacterial clearance rate was 92.86%(13/14) of patients. After treatment, the levels of WBC, SCr, BUN and PCT and body temperature in patients with urinary sepsis were significantly lower than those before treatment (P<0.05). After treatment, the ALT levels increased in 4 patients, and ALT levels of 3 cases were three times higher than that before treatment, but there was no significant difference in ALT level between before and after treatment in 31 patients (P>0.05). There were few adverse reactions in the course of treatment of patients. Conclusion Meropenem for injection has a definite effect on early empiric treatment of urinary sepsis. It has high bacterial clearance rate and clinical cure rate, with few adverse reactions and high clinical application value.
