摘 要:目的探讨新生儿感染性肺炎(NIP)应用免疫球蛋白治疗的临床效果。方法选择本院2016年1月至2018年10月收治的新生儿感染性肺炎患儿70例作为研究对象,纳入对象家属签署知情同意书愿意配合研究,以入组奇偶顺序分为两组,各35例,对照组(奇数)予以常规治疗方案处理,观察组(偶数)除了常规治疗方案,还加用免疫球蛋白治疗。评价两组患儿临床疗效,记录症状缓解时间(体温恢复正常、咳嗽消失、啰音消失)与住院时间,同时测定治疗前与治疗后免疫组化指标(IgG、CD3^+、CD4^+),予以比较分析。结果观察组总有效率高达94.28%,显著高于对照组74.28%(P<0.05);观察组患儿体温恢复正常时间、咳嗽消失时间、啰音消失时间、住院时间均显著短于对照组(P<0.05);两组患儿在治疗前IgG、CD3^+、CD4^+比较差异无统计学意义,治疗后观察组前述指标均明显高于对照组(P<0.05)。结论新生儿感染性肺炎患儿在常规治疗方案上加用免疫球蛋白治疗,一方面可以提高疗效,缩短症状缓解与住院时间,另一方面还可显著改善患儿的免疫组化指标,提高免疫能力,值得在临床应用。Objective To investigate the clinical effect of immunoglobulin in the treatment of neonatal infectious pneumonia(NIP).Methods 70 newborns with infective pneumonia admitted in our hospital from January 2016 to October 2018 were selected as the study objects. The subjects’ families signed informed consent to cooperate with the study. They were divided into two groups according to the order of parity and even, 35 cases each. The control group(odd number) was treated with routine treatment, and the observation group(even number) was treated with immunoglobulin in addition to routine treatment. Evaluate the clinical effect of the two groups, record the symptom remission time(body temperature return to normal,cough disappeared, rale IgG,CD3^+ and CD4^+ was measured before and after treatment. Results The total effective rate of the observation group was94.28%, which was significantly higher than that of the control group(74.28%)(P<0.05). The recovery time of body temperature, the time of cough disappearance, the time of rale disappearance and the time of hospitalization in the observation group were significantly shorter than those in the control group(P<0.05). There was no significant difference in IgG, CD3^+ and CD4^+ between the two groups before treatment. After treatment, the above indexes in the observation group were significantly higher than those in the control group(P<0.05). Conclusion The routine treatment of neonatal infectious pneumonia with immunoglobulin can improve the curative effect and shorten the time of symptom relief and hospitalization, on the other hand, it can significantly improve the immunocytochemical index of children with infectious pneumonia. It is worthy of clinical application to improve immune ability.
