热灌注化学治疗联合重组人p53腺病毒治疗肝癌腹腔积液临床研究

摘　　要：目的探讨热灌注化学治疗(简称化疗)联合重组人p53腺病毒治疗肝癌腹腔积液对患者恶性肿瘤相关物质群(TSGF)、癌胚抗原(CEA)、基质金属蛋白酶(MMPs)及血管内皮生长因子(VEGF)的影响。方法选取医院2017年1月至2018年1月收治的肝癌腹腔积液患者78例,依据治疗方式的不同分为两组,各39例。对照组予热灌注化疗,试验组予热灌注化疗联合重组人p53腺病毒注射液治疗。结果试验组TSGF、CEA、MMPs及VEGF水平显著低于对照组(P <0. 05);试验组总有效率为94. 87%,显著高于对照组的76. 92%(P <0. 05);两组不良反应发生率无显著差异(P> 0. 05)。结论热灌注化疗联合重组人p53腺病毒治疗肝癌腹腔积液可有效降低TSGF,CEA,MMPs,VEGF水平,提升疗效,且不良反应少。Objective To investigate the clinical efficacy of hyperthermic perfusion chemotherapy combined with recombinant human adenovirus p53 in the treatment of hepatocellular carcinoma ( HCC ) complicated with abdominal dropsy and its effect on tumor supplied group of factors ( TSGF ),carcinoembryonic antigen ( CEA ),matrix metalloproteinases ( MMPs ) and vascular endothelial growth factor ( VEGF ). Methods Totally 78 patients with HCC complicated with abdominal dropsy admitted to our hospital from January 2017 to January 2018 were selected and divided into two groups according to different treatment methods,39 cases in each group. The patients in the control group were treated with hyperthermic perfusion chemotherapy,while the patients in the experimental group were treated with hyperthermic perfusion chemotherapy combined with Recombinant Human Adenovirus p53 Injection. Results The levels of TSGF,CEA,MMPs and VEGF in the experimental group were significantly lower than those in the control group ( P < 0. 05 ). The total effective rate of the experimental group was 94. 87%,which was significantly higher than 76. 92% of the control group ( P < 0. 05 ). There was no significant difference in adverse reactions between the two groups ( P > 0. 05 ). Conclusion Hyperthermic perfusion chemotherapy combined with recombinant human adenovirus p53 can effectively reduce the levels of TSGF,CEA,MMPs and VEGF in the patients with HCC complicated with abdominal dropsy,and it can improve the clinical efficacy with fewer adverse reactions.
