多发性骨髓瘤日间病房管理模式的医疗成本探讨

摘　　要：目的:探讨多发性骨髓瘤(MM)患者便捷就医,降低医疗成本,减少院内感染发生率的治疗管理模式。方法:对2013-2017年在中南大学湘雅二医院血液科普通病房及日间病房完成四疗程及以上硼替佐米治疗的MM患者进行回顾性分析,探究两种不同管理模式下MM患者院内感染发生率、感染费用、治疗费用、结算自付费用及平均住院日之间的差异。共计92例患者纳入研究。结果:与普通病房组相比,日间病房组的MM患者院内感染率降低55.76%,感染费用下降100%,治疗费用(不含硼替佐米)下降93.78%,平均住院日每疗程缩短13天,自付费用下降68.38%。结论:新的治疗管理模式在降低医疗成本,减轻患者及医保负担方面具有明显的优势。Objective:To explore a better treatment management mode of patients with multiple myeloma(MM)to seek a more convenient medical treatment,reduce medical cost and lower the incidence of hospital infection.Methods:A retrospective analysis of patients with MM treated with bortezomib for four or more courses in general wards and day wards of Hematology department in the Second Xiangya Hospital of Central South University from 2013 to 2017 was conducted.The diff erence between MM under two diff erent management modes was investigated about the nosocomial infection rate,infection-related courses,treatment-related costs,out of pocket costs and average days of hospitalization.A total of 92 patients were enrolled in the study.Results:Compared with MM patients in the general ward,the rate of hospital infection of patients with MM in the day wards decreased by 55.7%,the infection-related costs decreased by 100%,the treatment-related costs decreased by 93.78%,the average hospital stay per treatment was shortened by 13 days and the fi nal self-pay decreased by 68.38%.Conclusion:New treatment management mode has obvious advantages in reducing medical costs and reducing the economic burden on patients and medical insurance.
