自体输血与同种输血对骨科患者免疫功能及氧合代谢水平分析

摘　　要：目的骨科患者应用自体输血、同种输血对氧合代谢、免疫功能指标的影响分析。方法选取2018年1月—2019年1月该院诊治90例骨科输血患者临床资料,按照随机数表分两组,对照组予以异体同种输血,研究组予以自体输血方案,分析两组患者氧合代谢、免疫功能指标及并发症情况。结果干预后,研究组SvO2(77.06±2.23)%、PvO2(75.23±2.46)mmHg比对照组高,LacV(1.30±0.12)mmol/L比对照组低(t=7.727 7、6.081 8、3.791 7,P<0.05);且研究组CD3+(52.76±4.23)%、CD4+(33.23±3.46)%等免疫指标比对照组高;研究组CD8+(28.71±6.28)%比对照组低,研究组免疫指标变化幅度比对照组少,差异有统计学意义(t=4.462 7、 8.892 5、4.337 0,P<0.05);研究组发生高热、泌尿感染等并发症4.44%比对照组24.44%少,比较差异有统计学意义(χ^2=7.282,P<0.057)。结论骨科患者应用自体输血相比异体同种输血方案,能有效改善患者氧合代谢指标,且免疫功能指标影响相对更小,并发症发生率少,具一定临床应用价值。Objective To analyze the effects of autologous blood transfusion and homologous blood transfusion on oxygenation metabolism and immune function in orthopedic patients. Methods The clinical data of 90 patients with orthopedic transfusion were diagnosed and treated in our hospital from January 2018 to January 2019. The patients were divided into two groups according to the random number table. The control group received allogeneic blood transfusion. The study group gave autologous blood transfusion protocol and analyzed the two groups of patients of oxygen metabolism, immune function indicators and complications. Results After intervention, SvO2 (77.06±2.23)% and PvO2 (75.23±2.46) mmHg were higher in the study group than in the control group, and LacV (1.30±0.12) mmol/L was lower than the control group(t=7.727 7, 6.081 8, 3.791 7,P<0.05);and the study group CD3+(52.76±4.23)%, CD4+(33.23 ± 3.46)% and other immune indicators higher than the control group;study group CD8+(28.71±6.28)% lower than the control group, the study group immune index change range, compared with the control group(t=4.462 7, 8.892 5, 4.337 0,P<0.05);the study group had 4.44% of high fever, urinary infection and other complications than the control group 24.44%, the difference was statistically significant (χ^2=7.282 7,P<0.05). Conclusion Autologous blood transfusion can improve the oxygenation index of patients with autologous blood transfusion. The immune function index is relatively less, the complication rate is less, and it has certain clinical application value.
