摘 要:目的探究无创正压通气应用于急诊重症支气管哮喘合并呼吸衰竭患者的临床疗效。方法该次实验对象全部选自该院急诊2017年3月—2019年1月期间收治的74例重症支气管哮喘合并呼吸衰竭患者,均分成37例患者的对照组和37例的研究组。对照组开展多种普通急诊治,在此基础上研究组开展无创正压通气治疗,分析两种处理方案对患者的影响。结果研究组和对照组在治疗后的PaCO2、SpO2、PaO2、IL-8、IL-17水平上,差异有统计学意义(t=12.701、8.123、7.216、12.662、2.061;P=0.000、0.000、0.000、0.000、0.043)。实验组的治疗总有效率为94.59%、脱机成功率为94.59%,、死亡率为0.00%,对照组分别为78.38%、75.68%、13.51%,差异有统计学意义(χ^2=4.163、5.232、5.362;P=0.041、0.022、0.021)。结论无创正压通气应用于急诊重症支气管哮喘合并呼吸衰竭患者的临床疗效较好,能够明显稳定患者血气指标,缓解多种炎性指标,减少死亡率。Objective To investigate the clinical efficacy of noninvasive positive pressure ventilation in patients with severe bronchial asthma complicated with respiratory failure. Methods All the subjects in this study were selected from 74 patients with severe bronchial asthma complicated with respiratory failure who were admitted to our hospital from March 2017 to January 2019. They were divided into the control group of 37 patients and the study group of 37 patients. The control group underwent a variety of general emergency treatments. On this basis, the research group conducted noninvasive positive pressure ventilation therapy and analyzed the effects of the two treatment regimens on patients. Results The levels of PaCO2, SpO2, PaO2, IL-8 and IL-17 in the study group and the control group were statistically significant (t=12.701, 8.123, 7.216, 12.662, 2.061;P=0.000, 0.000, 0.000, 0.000, 0.043). The total effective rate of treatment in the experimental group was 94.59%, the success rate was 94.59%, and the mortality rate was 0.00%. The control group was 78.38%, 75.68%, and 13.51%, respectively. The difference was statistically significant (χ^2=4.163, 5.232, 5.362;P= 0.041, 0.022, 0.021). Conclusion Non-invasive positive pressure ventilation is effective in patients with severe bronchial asthma complicated with respiratory failure. It can significantly stabilize the blood gas index, alleviate various inflammatory indexes and reduce mortality.
