摘 要:目的分析超声辅助定位对椎管内麻醉穿刺成功率提高的效果。方法选择2016年1月—2018年9月89例接受腹部及下肢手术治疗的患者,随机分为两组,45例接受超声辅助定位,视作观察组,44例采取传统解剖方法定位,视作对照组,比较两组定位质量、穿刺成功率。结果对照组穿刺一次成功率为68.18%,穿刺失败率13.64%,观察组穿刺一次成功率86.67%,穿刺失败率为2.22%(χ^2=4.363 2、3.999 7,P<0.05)。观察组不良反应发生率为4.44%,对照组不良反应发生率18.18%(χ^2=4.209 5,P<0.05);观察组穿刺次数少于对照组,定位时间长于对照组,穿刺时间、操作时间短于对照组(P<0.05),总时间与对照组差异无统计学意义(P>0.05)。结论超声辅助定位有助于明显提高椎管内麻醉穿刺一次成功率,缩短穿刺成功所需时间,有更好的应用价值。Objective To analyze the effect of ultrasound-assisted positioning on the success rate of spinal anesthesia puncture. Methods Eighty-nine patients who underwent abdominal and lower extremity surgery from January 2016 to September 2018, were randomly divided into two groups. 45 patients underwent ultrasound-assisted localization and were treated as observation group. 44 patients underwent traditional anatomical methods as a control group, the positioning quality and puncture success rate of the two groups were compared. Results The success rate of puncture was 68.18%, the puncture failure rate was 13.64%, the success rate of puncture was 86.67%, and the puncture failure rate was 2.22%(χ^2=4.363 2, 3.999 7, P<0.05). The incidence of adverse reactions in the observation group was 4.44%, and the incidence of adverse reactions in the control group was 18.18%(χ^2=4.209 5, P<0.05). The number of punctures in the observation group was less than that in the control group, and the localization time was longer than that in the control group. The puncture time and operation time were shorter than the control group(P<0.05). The total time was not statistically significantly different from the control group(P>0.05). Conclusion Ultrasound-assisted positioning can significantly improve the success rate of spinal anesthesia puncture and shorten the time required for successful puncture. It has better application value.
