摘 要:目的分析CT与MRI在检查小脑血管母细胞瘤方面的诊断效果,从其影像学特征中分析影像诊断的价值。方法选择2016年8月-2017年8月该院收治的经手术病理确诊为小脑血管母细胞瘤的54例患者作为该次研究对象,随机将其分为两组,每组27例。一组行CT检查,一组行MR检查。通过比较两组患者在检查结果方面的影像学特征,来分析两组诊断的效果。结果该次研究结果表明,CT组在特异度、灵敏度与诊断准确率上均要低于MR组。特异度方面,MR组为85.2%,而CT组为74.1%;在灵敏度方面,MR组的92.6%要优于CT组的70.4%;在诊断准确率方面,MR组的92.6%同样要高于CT组的66.7%。两组患者各项数据对比差异有统计学意义(χ2=3.805、8.540、8.551,P<0.05)。另外,MR组中主要表现为圆形大囊小结节型,边缘光滑且界限清晰,TI现实囊液信号均匀,囊内信号略低,其中有少数患者表现为边缘不规则与团块状分布。CT组中主要表现为囊性区密度高于脑脊液,实质内核中央出现部分低密度坏死区域。结论在小脑血管母细胞瘤的影像诊断过程中,MR的临床应用价值更加突出,漏诊情况控制较好,灵敏度与特异度有所保障,影像学结果表现清晰,可以成为今后相关疾病诊断的主要方式。Objective To analyze the diagnostic effect of CT and MRI in the examination of cerebellar hemangioblastoma, and to analyze the value of image diagnosis from its imaging characteristics. Methods A total of 54 patients who admitted to our hospital from August 2016 to August 2017 with cerebellar hemangioblastoma diagnosed by surgery and pathology were selected. They were randomly divided into two groups, 27 cases in each group. One group received CT examination and one group received MR examination. By comparing the imaging features of the examination results of the two groups, the diagnostic effects of the two groups were analyzed. Results The results of this study showed that the specificity, sensitivity and diagnostic accuracy of the CT group were lower than those of the MR group. The specificity was 85.2% in the MR group and 74.1% in the CT group. The sensitivity was 92.6% of MR group, which was better than that of CT group 70.4%. The diagnostic accuracy was 92.6% in the MR group, which was also higher than that in the CT group 66.7%. There were significant differences in the data between the two groups, with statistical significance(χ2=3.805,8.540, 8.551,P<0.05). In addition, in the MR group, the main manifestations were round cystic nodules, with smooth and clear edges, uniform TI realistic cystic fluid signal, and slightly low cystic signal. And there were a few patients showing irregular and lump-like distribution of edges. In the CT group, the density of cystic area was higher than that of cerebrospinal fluid, and some areas of low-density necrosis were found in the center of parenchymal kernel.Conclusion In the process of imaging diagnosis of cerebellar hemangioblastoma, the clinical application value of MR is more prominent, with few missed diagnosis. The sensitivity and specificity are guaranteed, and the imaging results are clear, which can become the main way of diagnosing related diseases in the future.
