我院静脉用药调配中心不合理医嘱调查分析

摘　　要：目的:了解我院住院医嘱不合理用药情况,为临床医生开具医嘱提供参考,促进临床合理用药。方法:通过我院PIVAS使用的智慧园软件系统提取2018年7月~2019年2月审核的全部静脉用药医嘱,对不合理用药医嘱进行分析。结果:经统计,本次共提取34237组医嘱,不合理用药医嘱299组(0.87%)。溶媒选择不适宜118组(39.5%),配伍禁忌38组(12.7%),给药浓度不适宜35组(11.7%),给药剂量不适宜30组(10.0%),用药频次不适宜23组(7.7%),未分组开具医嘱22组(7.3%),医嘱未联合11组(3.7%),重复发送医嘱9组(3.0%),给药途径不适宜6组(2.0%),TPN处方组成不合理5组(1.7%),有禁忌症2组(0.7%)。结论:我院住院医嘱用药合理率比较高。对医嘱的开具施行信息化管理,完善审方药师的专业知识结构,减少了不合理医嘱和药物不良反应。Objective:To understand the unreasonable use of medical advice in our hospital, so as to provide reference for clinicians to prescribe medical advice and further promote rational clinical use. Methods:All intravenous medication orders reviewed From July 2018 to February 2019 were extracted and analyzed through the intelligent software system used by PIVAS in our hospital, and unreasonable medication orders were extracted and analyzed. Results:According to statistics, a total of 34,237 groups of medical orders were extracted this time, 299 groups (0.87%) of which had unreasonable medication orders. Among them, 118 groups (39.5%) were not suitable for solvent selection, compatibility taboo 38 groups (12.7%), the dose concentration is not suitable 35 groups (11.7%), 30 groups(10.0%) with inappropriate dose, the drug use frequency was not suitable for the 23 groups(7.7%),22 groups(7.3%)prescription were no group, no prescription combined with 11 groups (3.7%), 9 groups(3.0%) of repeated delivery orders, the way of unsuitable medical for 6 groups(2.0%), unreasonable TPN prescriptions in 5 groups (1.7%), 2 groups with contraindications(0.7%). Conclusion:The rational rate of prescribed drugs in our hospital is relatively high.The informationized management of the prescribing of medical advice can improve the professional knowledge structure of the pharmacists who review prescriptions and reduce the occurrence of unreasonable medical advice and adverse drug reactions.
