摘  要:目的探讨血清中脑钠肽和醛固酮水平与慢性心功能不全患者治疗效果及临床预后的关系。方法选取2016年11月-2017年11月在本院就诊的120例慢性心功能不全的患者作为研究对象(观察组),并选取30例健康体检者作为对照组。120例患者均采用慢性心功能不全的综合治疗,均在治疗前检测血清中脑钠肽(BNP)和醛固酮(ALD)水平,评价BNP和ALD水平与患者治疗效果的关系,并采用Logistic回归分析探讨患者血清中BNP和ALD水平与患者预后的关系。结果观察组患者BNP、ALD水平明显高于对照组(P <0.05);BNP低水平组及ALD低水平组LVEDP、RVED、m PAP水平均低于高水平组(P <0.05),而LVEF水平均高于高水平组(P <0.05)。BNP低水平组及ALD低水平组总有效率均高于对应高水平组(P <0.05)。Logistic回归分析结果显示,LVEF、BNP、ALD是患者心血管事件的独立风险因素(P <0.05)。结论 BNP和ALD与CRF治疗效果密切相关,是患者心血管事件发生的独立危险因素。Objective To explore the relationship between serum brain natriuretic peptide( BNP), aldosterone( ALD) level and the therapeutic effect and prognosis of patients with chronic heart failure. Methods 120 patients with chronic cardiac insufficiency in our hospital during November 2016-November 2017 were selected as the subjects. 120 patients were all treated with chronic cardiac insufficiency, all the serum levels of BNP and ALD were detected before treatment. The relationship between BNP, ALD levels and patient outcomes was evaluated, and Logistic regression analysis was used to investigate the relationship between serum BNP, ALD levels and prognosis. Results The levels of BNP and ALD in the observation group were significantly higher than those in the control group (P <0.05). The levels of LVEDP, RVED and mPAP in the low BNP group and the low ALD group were lower than those in the high level group ( P <0. 05 ), while the LVEF level was higher in the BNP, ALD high level group ( P < 0.05 ). The total effective rate of the BNP low-level group and the ALD low-level group was higher than that of the high-level group, with the differences statistically significnat ( P <0.05). Logistic regress io n analysis showed that LVEF ( P =0.03), BNP ( P =0.04), and ALD ( P = 0.02) were independent risk factors for cardiovascular events in patients. Conclusion BNP and ALD are closely related to the therapeutic effect of CRF, and are independent risk factors for cardiovascular events in patients.
