摘 要:目的观察梗阻性急性肾损伤(AKI)患者血清血红素加氧酶1(HO-1)的水平变化,并探讨其临床意义。方法选取33例梗阻性AKI患者(AKI组)及30例健康体检者(健康对照组),采用酶联免疫吸附法检测两组血清HO-1、肾损伤因子(KIM-1)等指标。采用Spearman相关分析血清HO-1、KIM-1水平的相关性;绘制受试者工作特征(ROC)曲线评价HO-1对梗阻性AKI的诊断价值;用二分类Logistic回归分析梗阻性AKI发病的影响因素。结果AKI组血清HO-1、KIM-1水平高于健康对照组(P均<0.01)。AKI组血清HO-1水平与血清KIM-1水平呈正相关(r=0.596,P<0.01)。HO-1对AKI的诊断界值为0.60pg/mL,其ROC曲线下面积为0.869,敏感度和特异度分别为81.8%、77.8%。血清HO-1、KIM-1水平升高是梗阻性AKI发生的危险因素。结论梗阻性AKI患者血清HO-1水平升高,其诊断梗阻性AKI的灵敏度和特异度较高,HO-1水平升高的患者易发生梗阻性AKI。Objective To observe the changes of serum heme oxygenase 1 (HO-1) level in patients with obstructive acute kidney injury (AKI) and to explore its clinical significance. Methods Thirty-three obstructive AKI patients (AKI group) and 30 healthy controls (control group) were recruited.Serum HO-1 and KIM-1 were measured by enzyme linked immunosorbent assay (ELISA).The relationship between HO-1 level and KIM-1 level were tested by Spearman correlation analysis.We drew the ROC curve to explore the predictive value of serum HO-1 level for obstructive AKI,and evaluated the influencing factors of AKI by binary Logistic analysis. Results The levels of HO-1 and KIM-1 were higher in the AKI group than in the control group (both P <0.01).Serum HO-1 level was positively correlated with KIM-1 level in AKI group ( r =0.596,P <0.01). The diagnostic cut-off value of HO-1 for AKI was 0.60 pg/mL,area under ROC of HO-1 was 0.869,with a sensitivity of 81.8% and a specificity of 77.8%.The elevated levels of serum HO-1 and KIM-1 were risk factors for obstructive AKI.Conclusion The serum HO-1 level increases in patients with obstructive AKI,with high sensitivity and specificity for the diagnosis of obstructive AKI,and patients with elevated HO-1 level are prone to obstructive AKI.
