摘  要:目的颅脑超声测量新生儿矢状面胼胝体长度及小脑蚓部长度,探讨新生儿脑发育与母体分娩孕周及出生体重的相关性。方法 选取2015年10月至2018年6月首都医科大学附属北京潞河医院新生儿430例为研究对象,均在出生3d内进行颅脑超声检查,测量矢状面胼胝体长度及小脑蚓部长度,分析其与母体分娩孕周及出生体重的相关性。结果 母体分娩孕周29~33周、34~36周及37周以上新生儿颅脑矢状面测量的胼胝体长度、小脑蚓部长度及出生体重比较差异有统计学意义(P<0.01),且母体分娩孕周34~36周及37周以上新生儿大于29~33周,37周以上大于34~36周(P<0.01)。Spearman相关性分析显示:新生儿颅脑矢状面测量的胼胝体及小脑蚓部长度与母体分娩孕周及出生体重呈正相关(P<0.01)。结论 颅脑超声适用于新生儿颅脑早期监测脑发育情况,新生儿颅脑矢状面测量的胼胝体长度、小脑蚓部长度与母体分娩孕周和出生体重呈正相关。Objective To measure sagittal corpus callosum length and cerebellar vermis length in neonates by craniocerebral ultrasonography,to explore the correlation of neonatal brain development with gestational age and birth weight. Methods A total of 430 neonates in Beijing Luhe Hospital from Oct. 2015 to Jun. 2018 were included in the study.All neonates underwent brain ultrasound examination within 3 days of birth.The sagittal corpus callosum length and the cerebellar vermis length were measured to analyze the correlation with gestational age and birth weight. Results There were significant differences in the length of corpus callosum, the length of cerebellar vermis measured by sagittal plane measurement,and birth weight between neonates with maternal gestational age 29-33 weeks,34-36 weeks and more than 37 weeks( P < 0.01). The neonates at 34-36 weeks and over 37 weekshad higher indexes than neonates of 29-33 weeks,and those above 37 weeks had higer indexes than the 34-36 weeks( P <0.01). Spearman correlation analysis showed that the length of the sagittal corpus callosum and the length of the cerebellar vermis were positively correlated with the gestational age and birth weight( P <0.01). Conclusion Craniocerebral ultrasonography has a significant value for early brain monitoring of neonates.The length of corpus callosum and the length of cerebellum vermis measured on sagittal plane of neonate ′s skull are positively correlated with gestational age and birth weight.
