摘  要:目的探讨糖化血红蛋白(HbA1c)水平在老年2型糖尿病(T2DM)患者中诊断冠心病的价值。方法选择老年2型糖尿病患者198例,均行冠脉造影检查。其中合并冠心病患者101例、非冠心病患者97例。检测两组患者HbA1c水平,根据HbA1c水平,将合并冠心病患者分成A组,HbA1c<6.5%。B组7.5%≥HbA1c≥6.5%:C组HbA1c>7.5%。统计冠脉病变支数及Gensini积分。结果老年T2DM合并冠心病患者中的HbA1c水平较未合并冠心病者明显增高(P<0.05),且随着HbA1c水平升高,冠状动脉狭窄越严重,两组Gensini积分差异有统计学意义(P<0.05)。结论对于老年2型糖尿病患者,HbA1c水平可在一定程度上判断患者冠状动脉狭窄程度。Objective To explore the value of glycosylated hemoglobin(HbA 1c)level in the diagnosis of coronary heart disease in elderly patients with type 2 diabetes mellitus.Methods 198 elderly patients with type 2 diabetes mellitus were selected for coronary angiography,including 101 patients with coronary heart disease and 97 patients without coronary heart disease.HbA 1c levels of patients in two groups were detected.According to HbA 1c levels,patients with coronary heart disease were divided into group A with HbA 1c<6.5%,group B with 7.5%≥HbA 1c≥6.5%and group C with HbA 1c>7.5%.And the number of coronary artery lesions and Gensini integral were calculated.Results The level of HbA 1c in elderly patients with T2DM complicated with coronary heart disease was significantly higher than that of those without coronary heart disease,and the higher the level of HbA 1c was,the more serious the coronary artery stenosis was,with statistically significant differences between two groups in Gensini integral.Conclusion HbA 1c level is related to the stenosis degree of coronary artery in elderly patients with type 2 diabetes mellitus to some extent.
