摘 要:目的:分析寒痰郁肺型毛细支气管炎患儿中施予射干麻黄汤加减与布地奈德联用的疗效。方法:抽取2015年12月至2017年9月间进入本院儿科确诊的143例患毛细支气管炎患儿,按随机数字表法分为治疗组73例给予射干麻黄汤加减疗法与布地奈德联用,对照组70例给予布地奈德单治,回顾性比较其疗效、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-8(IL-8)、降钙素原(PCT)及C-反应蛋白(hs-CRP)。结果:治疗组有效率97.26%,比对照组的84.29%更高;治疗组PCT(0.41±0.11)ng/ml、hs-CRP(0.50±0.09)mg/dl,比对照组的PCT(0.55±0.19)ng/ml、hs-CRP(0.65±0.20)mg/dl更低。治疗组患儿心率指数达到健康水平所用时间、气促现象消失时间、哮鸣音现象消失时间以及湿罗音现象消失时间均比对照组短,治疗组CD4^+、CD4^+/CD8^+比对照组高,而CD8^+比对照组低,治疗组TNF-α、IL-6及IL-8均比对照组低。结论:儿科给予毛细支气管炎患儿专业治疗时以射干麻黄汤加减疗法与布地奈德联用效果显著,值得临床推广。Objective: To analyze the effectiveness of the combination of Shegan Mahuang Decoction plus budesonide in the clinical treatment of children with Cold and stagnation bronchiolitis. Methods: A total of 143 patients with bronchiolitis were selected and admitted to our pediatric definite children between December 2015 and September 2017. Randomly divided into groups: 73 patients in the treatment group were treated with Shegan Mahuang Decoction and Budesonide. In the control group, 70 patients were treated with budesonide alone, and their efficacy, the levels of TNF-α, IL-6, IL-8, PCT values, hs-CRP values were retrospectively compared. Results: The effective rate of the treatment group was 97.26%, which was higher than that in the control group of 84.29%. The PCT value in the treatment group after treatment was(0.41±0.11) ng/ml, hs-CRP value(0.50±0.09) mg/dl, which were lower than those of the control group of PCT value(0.55±0.19) ng/ml, hs-CRP value(0.65±0.20) mg/dl. The time taken for the heart rate index to reach the healthy level, the disappearance time of the shortness of breath, the disappearance the wheezing phenomenon, and the disappearance time of the wet rales were all shorter in the treatment group than those of the control group. The CD4^+ and CD4^+/CD8^+ in the treatment group were higher than those of the control group, while CD8^+ was lower than the control group. In the treatment group, tumor necrosis factor-alpha(TNF-alpha) and interleukin-6(IL-6) were also treated. Both IL-8 were lower than those of the control group. Conclusion: When pediatrics are used to treat children with bronchiolitis, the combination of Shegan Mahuang Decoction plus budesonide has significant effects and is worthy of promotion.
